Background:Anti-Mullerian hormone (AMH) is considered as a good marker for quantitative evaluation of ovarian response to the stimulation during assisted reproductive technology cycles.Objective:To evaluate the association between serum AMH level and embryo morphokinetics using time-lapse imaging and intracytoplasmic sperm injection (ICSI) outcomes in women with polycystic ovarian syndrome (PCOS).Materials and Methods: We evaluated a total of 547 embryos from 100 women underwent ICSI cycles; 50 women with PCOS and 50 women with tubal factor infertility. Serum AMH level was measured in all participants. Time-laps records were annotated for time to pronuclear fading (tPNf), time to 2-8 cells (t2-t8), reverse cleavage, direct cleavage, and also for the presence of multinucleation. Results:AMH was negatively correlated with t5, t8, and the third cell cycle (p=0.02, p=0.02, and p=0.01; respectively) in PCOS group. AMH had no correlation with embryo kinetics in infertile women with tubal factor infertility. Moreover, AMH level is similar between embryos with and without direct cleavage as well as reverse cleavage and Multinucleation in both groups. The Receiver operating characteristic curves analyses indicated that AMH was not an accurate predictor of clinical pregnancy as well as a live birth (AUC=0.59 [95% CI, 0.42-0.76]) in PCOS women. However, in the women with tubal factor infertility AMH showed a fair prediction value for clinical pregnancy (AUC=0.64 [95% CI, 0.48-0.82]) along with the live birth (AUC=0.70 [95% CI, 0.55-0.85]).Conclusion: Some of the time-lapse embryo parameters may be related to the AMH concentration. However, AMH is not an accurate tool to predict the ICSI outcomes in PCOS women.

Background Granulocyte-macrophage colony-stimulating factor (GM-CSF) expressed in the human reproductive system, holds a pivotal role in the reproductive processes. This study investigates the in vitro effect of GM-CSF on the testicular sperm of obstructive azoospermia (OA) patients and assesses the effectiveness of GM-CSF‐supplemented sperm media in Intracytoplasmic sperm injection (ICSI) outcomes. Methods and Results Following testicular sperm extraction from 20 patients diagnosed with OA, each sample was divided into two parts: the experimental samples were incubated with the medium containing 2 ng/ml GM-CSF at 37°C for 60 min, and control samples were incubated with medium without GM-CSF. Subsequently, the oocytes retrieved from the partner were injected with sperms from treatment and the control groups. The sperm parameters ( motility, viability), the expression level of sperm motility-related genes (PIK3R1, PIK3CA, and AKT1 ), and sperm energy metabolism-related genes (GLUT1, GLUT3, and GLUT14) were assessed. Furthermore, the fertilization and cleavage rates and embryo quality were evaluated. Supplemented testicular sperm with GM-CSF significantly increased motility parameters, the mRNA expression of PIK3R1, AKT1, and GLUT3 compared to the non-treated group (p < 0.05). However, no significant differences in mRNA expression of PIK3CA, GLUT1, or GLUT14 were identified. Based on ICSI outcomes, the GM-CSF treatment group exhibited significantly higher fertilization rates (p = 0.027), cleavage rates (p = 0.001), and the proportion of good-quality embryos (p = 0.002) compared to the control group. Conclusions GM-CSF increased gene expression related to motility and energy metabolism pathway and effectively had a positive effect on the motility of testis-extracted spermatozoa and, consequently yielding positive clinical outcomes.

Background: A significant association between endometrial vascularity and pregnancy has been shown in previous research, while poor vascularization was attributed to repeated implantation failure (RIF). One possible approach to enhance angiogenesis for successful implantation is endometrial scratching (ES). Objective: The purpose was to investigate endometrial responses to scratching by profiling angiogenesis-related gene expression in unexplained RIF participants. Materials and Methods: In this randomized controlled trial study, 20 infertile women with unexplained RIF were assigned to 2 groups by the balanced block randomization method (n = 10/each group): the intervention group (group A) (who received ES in the follicular phase) and the control group (group B). Endometrial biopsy was performed in the secretory phase. Gene expression profiling was performed using a polymerase chain reaction-array kit for humanangiogenic growth factors. The implantation and clinical pregnancy rates were also assessed. Results: Among the angiogenesis-promoting genes, FGF1, FGF13, FGF2, TGFA, ANG, ANGPT1, and VEGFA were significantly upregulated (p < 0.05). IL12A (an angiogenesis-inhibiting cytokine) was significantly upregulated (p < 0.01). In contrast, 15 genes with angiogenesisrelated functions, including CXCL11, CXCL13, CXCL3, CXCL5, CXCL6, EREG, FIGF, FST, IL10, LEP, PPBP, PROK1, RHOB, TNF, and TYMP, were downregulated after ES. No significant differences were observed between the intervention (group A) and control (group B) groups in terms of implantation (43.75% vs. 28.57%) or clinical pregnancy rates (75% vs. 57.1%). Conclusion: ES induced significant alterations in the expression of angiogenesis-related genes, with notable up/downregulation of key angiogenic/antiangiogenic factors. These findings enhance our understanding of the molecular responses triggered by ES, underscoring the potential influence of ES on the complex processes of angiogenesis crucial for implantation.
