ÖzetAmaç: Bu çalışma ile kliniğimizde uygulanan tüp torakostomi (TT) işlemleri-nin sonuçları araştırılmıştır. Gereç ve Yöntem: Aralık 2009 -Temmuz 2013 arasında Çanakkale Onsekiz Mart Üniversitesi Tıp Fakültesi Göğüs Cerrahisi Kliniği tarafından TT uygulanan 130 hastanın kayıtları retrospektif olarak incelendi. Bulgular: TT uygulanan 130 hastanın 102'si erkek (%78) 28' i (%22) kadındı. Hastaların ortalama yaşı 58.9±20.4 yıldı. Hastaların %19' unu (n:25) Göğüs Cerrahisi servisinin hastaları oluştururken %81' ini (n:105) dış servislerden tarafımıza konsülte edilen hastalar oluşturmaktaydı. TT endikasyonları arasında en sık rastlanan neden plevral efüzyondu (34,6%, n:45). Çeşitli nedenlerle plevral efüzyonu olan hastaların (%63, n:84) ortalama drenaj miktarı 1462.80±1054.46 ml olarak bulundu. Çeşitli nedenlerle pnömotoraks olan hastaların (%37, n:46) ortalama hava drenaj süresi 5.8±3.1 gün olarak bulundu. Kliniğimizde TT uygulanan 130 hastanın sadece bir tanesinde komplikasyon gelişti (%0,8). Bu komplikasyonda intraparankimal dren konulması idi. Tartışma: Genellikle acil koşullarda uygulanmak zorunda kalınan bu cerrahi girişimin potansiyel birçok ciddi komplikasyonunun da olması Göğüs Cerrahisi kliniklerindeki asistan eğitiminin önemini arttırmaktadır. Doğru endikasyonda, doğru yerden, cerrahi prensiplere uygun olarak takılan ve komplikasyon oranları çok düşük olan TT, Göğüs Cerrahlarının bulundukları hastanedeki önemini daha da arttıran ve diğer klinikler ile ilişkilerini geliştirmede önemli rol oynayan cerrahi bir işlemdir. Material and Method: The medical records of 130 patients to whom CTT were performed by the Thoracic Surgery Clinics of the Çanakkale Onsekiz Mart University from December 2009 to July 2013 were retrospectively analyzed. Results: 102 patients (78%) out of 130 were male and 28 (22%) were women. Mean age of the patients were 58.9±20.4. 19% (n:25) of these patients were diagnosed at our clinics and the remaining 81% (n:105) were consultated from other clinics The most common indication of CTT was pleural effusion (35%, n:45). The mean fluid drainage of the patients with pleural effusions for various reasons (63%, n:84) were 1462.80±1054.46. The mean air drainage time for the patients with pneumothorax for various reasons (37%, n:46) were 5.8±3.1 days. There was only one complication (0,8%) in these 130 patients which was the intraparanchymal drain placement. Discussion: As a procedure that is usually performed under emergency conditions CTT has a high potential for development of serious complications and thus increasing the importance of resident doctor training in Thoracic Surgery clinics. Malignant pleural effusion (MPE) may be an indication for palliative treatment to improve the quality of life and allow patients to leave the hospital. The most common treatment for MPE is drainage of the pleural cavity followed chemical pleurodesis. The aim of the study was to compare the efficacy of Bleomycin and Doxycycline for pleurodesis. Material and methods. Between 1996 and 2006, 105 patients (aged 30-84) were treated in the Wrocław Thoracic Surgery Centre for MPE using drainage of the pleural cavity followed by Bleomycin or Doxycycline for chemical pleurodesis. Lung cancer was diagnosed in 53 patients (49.5%), other malignancy in 49 patients (46.7%), and the primary focus was not established in 4 (3.8%) patients. Patients were divided into two groups: group I (n=56) consisted of patients who underwent pleurodesis with Bleomycin (60 mg), and group II (n=49) consisted of patients treated with Doxycycline (1 g or 0,5 g). Patients were divided into two subgroups. Subgroup A (n=31) included patients who died within the first month, and subgroup B (n=74) included patients who lived longer. Results. In subgroup A, total remission was achieved in 100% of group I -patients and 78.6% of group II -patients, respectively. There were no statistically significant differences (SSDs) between groups (p>0.05). In subgroup B, remission was analyzed after 1, 3, 6, and 9 months. Total remission after 9 months was achieved in 84.6% and 77.1% of patients in groups I and II, respectively. There were no statistical differences between groups. The median mean rate survival in groups I and II was 251 and 242 days, respectively (p>0.05). Drainage complications and side effects were analyzed. Conclusions. Better results were achieved following the use of Bleomycin than Doxycycline, but there were no SSDs between groups, nor were three any SSDs in survival length between groups. The most common side effect was pain, which occurred more frequently after treatment with Doxycycline than Bleomycin.

Рассмотрены современные подходы к лечению больных с первичным и метастатическим поражением плевры. Пред-ставлен опыт применения в МНИОИ им. П.А. Герцена методики внутриплевральной гипертермической химиотерапии с препаратом цисплатин у 7 больных с опухолевым поражением плевры. Отражены методология процедуры, особенности анестезиологического пособия, проанализирован характер осложнений. Приведено наблюдение успешного применения методики у пациента с мезотелиомой плевры. Ключевые слова: первичное и метастатическое поражение плевры, внутриплевральная гипертермическая химиотерапия.The paper considers approaches to treating patients with primary pleural malignancy and metastatic involvement. It presents experience in using cisplatin in the procedure of intrapleural hyperthermic chemotherapy in 7 patents with pleural tumors. It shows the methodology of the procedure and the specific features of anesthetic support and analyzes the nature of complications. The authors describe a case of successful use of the procedure in a patient with pleural mesothelioma.Keywords: primary pleural malignancy and metastatic involvement, intrapleural hyperthermic chemotherapy.

Abstract: These results indicate that talc slurry is as effective as bleomycin in preventing early recurrence of malignant pleural effusions. Pleurodesis with talc instead of bleomycin can result in significant cost savings.
