Objective: The aim of this paper was to investigate the communication attitude of Slovenian preschool children who stutter by means of the Communication Attitude Test for Preschool and Kindergarten Children who Stutter (KiddyCAT), and to determine whether differences in communication attitude existed among preschool children who stutter, and their peers who are fluent speakers. In addition, the test's discriminant value, its internal reliability and the factors underpinning the Slovenian version of the KiddyCAT were explored. Methods and Procedure:Data were gathered on a normative sample of 49 preschool children who stutter and 74 who do not stutter. The children were divided into two subgroups according to age: a ''younger'' (3 to 4,4 years old) and ''older'' (4,5 to 6 years old) group. Outcomes and Results:Results showed that preschool children who stutter scored statistically significantly higher on the KiddyCAT than preschool children who do not stutter. The effect size was large. A mean increase in scores among the preschool children who stutter was observed, but was not statistically significant. An opposite observation, again not significant, was made for preschool children who do not stutter. Also gender did not affect the test results. The test is internally reliable and has construct validity. Conclusion and Implications:The KiddyCAT is a useful tool for early detection of negative communication attitude of preschool children who stutter and helps in targeting cognitive-related stuttering intervention goals.

Motor balance in developmental stuttering (DS) was investigated with Transcranial Magnetic Stimulation (TMS), with the aim to define novel neural markers of persistent DS in adulthood. Eleven DS adult males were evaluated with TMS on tongue primary motor cortex, compared to 15 matched fluent speakers, in a “state” condition (i.e. stutterers vs. fluent speakers, no overt stuttering). Motor and silent period thresholds (SPT), recruitment curves, and silent period durations were acquired by recording tongue motor evoked potentials. Tongue silent period duration was increased in DS, especially in the left hemisphere (P<0.05; Hedge’s g or Cohen’s dunbiased = 1.054, i.e. large effect size), suggesting a “state” condition of higher intracortical inhibition in left motor cortex networks. Differences in motor thresholds (different excitatory/inhibitory ratios in DS) were evident, as well as significant differences in SPT. In fluent speakers, the left hemisphere may be marginally more excitable than the right one in motor thresholds at lower muscular activation, while active motor thresholds and SPT were higher in the left hemisphere of DS with respect to the right one, resulting also in a positive correlation with stuttering severity. Pre-TMS electromyography data gave overlapping evidence. Findings suggest the existence of a complex intracortical balance in DS tongue primary motor cortex, with a particular interplay between excitatory and inhibitory mechanisms, also in neural substrates related to silent periods. Findings are discussed with respect to functional and structural impairments in stuttering, and are also proposed as novel neural markers of a stuttering “state” in persistent DS, helping to define more focused treatments (e.g. neuro-modulation).

Objectives: The present study aimed to compare the communicative attitudes of stuttering groups with the general population using a meta-analysis and to examine the patterns of change in communicative attitudes across different age groups: pre-school, school-age, and adults. Methods: The author conducted a meta-analysis to compare communication attitude scores between people who stutter (PWS) and people who do not stutter (PWNS) from 2000 to June 2022 using domestic and foreign databases. 15 articles meeting the selection criteria were included, with 16 data points extracted. The study aimed to determine the overall effect size of communication attitude scores between groups and to analyze age as a moderator. Results: The overall effect size of the communication attitude scores between the two groups showed a large effect size (g = 2.507) and was statistically significant. Meta ANOVA analysis of group communication attitude scores by age category showed that there was a significant difference in effect size between the three age categories (χ2 = 13.53, p < .01). Post-hoc analysis was performed, and there was no significant difference in effect size between the pre-school and school-age groups (χ2 = 0.72, p = .40). A large effect size and a statistically significant difference were observed between the school-age and adult groups (χ2 = 5.95, p < .05), and a very large effect size and a statistically significant difference were observed between the pre-school and adult groups (χ2=13.52, p < .01). Conclusion: The research findings indicate that individuals who stutter exhibited more negative communicative attitudes compared to the non-stuttering group across all age groups. Particularly during adulthood, the negative communicative attitudes were more pronounced when compared to the other two age groups (pre-school and school-age). Therefore, when providing treatment for stuttering, it is important to not only focus on external behaviors but also incorporate activities that address age-appropriate communicative attitudes.
