SUMMARYThe present study was designed to investigate the incidence of benign joint hypermobility syndrome (BJHMS) in mitral valve prolapse (MVP) and the correlation between the echocardiographic features of the mitral valve and elastic properties of the aortic wall and Beighton hypermobility score (BHS) in patients with MVP and BJHMS.Fourty-six patients with nonrheumatic, uncomplicated, and isolated mitral anterior leaflet prolapse (7 men and 39 women, mean age; 26.1 ± 5.9) and 25 healthy subjects (3 men and 22 women, mean age, 25.4 ± 4.3) were studied. Patients were divided into two groups according to their BHS (group I, MVP+BJHMS; group II, MVP-BJHMS). Individuals with accompanying cardiac or systemic disease were excluded. Echocardiographic examination was performed in all subjects. The presence of BJHMS was evaluated according to Beighton's criteria.The incidence of BJHMS in patients with MVP was found to be significantly higher than that of controls (45.6%, (21/46) vs 12% (3/25), P < 0.0001). Group I (MVP + BJHMS) had significantly increased anterior mitral leaflet thickness (AMLT, 3.4 ± 0.4 vs 3.1 ± 0.3; P < 0.005), maximal leaflet displacement (MLD, 2.4 ± 0.4 vs 1.7 ± 0.4; P < 0.005), and degree of mitral regurgitation (DMR, 17.1 ± 7.2 vs 11.2 ± 4.4; P < 0.01) compared to group II. However, the index of aortic stiffness (IAOS) was found to be lower (17.6 ± 6.9 vs 23.9 ± 7.6; P < 0.005) and aortic distensibility (AOD) to be higher (0.0035 ± 0.007 vs 0.0024 ± 0.005; P < 0.005) in group I. There was a significant correlation between AMLT, MLD and DMR, and BHS (r = 0.57/P = 0.007, r = 0.55/P < 0.009, r = 0.51/P < 0.01, respectively). A relação da hipermobilidade com a redução da densidade mineral óssea ainda é desconhecida. Existem relatos de osteoporose em associação com doenças genéticas que cursam com hipermobilidade articular. O nosso objetivo foi detectar a possível associação entre hipermobilidade articular e alterações na densidade mineral óssea (DMO) em crianças com e sem dor múscu-lo-esquelética. Casuística e métodos:foram avaliadas 93 crianças, com idade entre 5 e 10 anos, quanto à presença de hipermobilidade articular e quanto à presença de dor músculo-esquelética, através de questioná-rio dirigido aos pais. Todas as crianças realizaram densitometria óssea de coluna lombar ao nível das vértebras L2-L4.Resultados: as crianças foram distribuídas de acordo com a presença ou não de hipermobilidade articular associada ou não à dor músculo-esquelética: 29 (31,2%) com hipermobilidade e com dor músculo-esquelética, 20 (21,5%) com hipermobilidade e sem dor, 22 (23,6%) sem hipermobilidade e com dor e 22 (23,6%) sem hipermobilidade e sem dor (grupo controle). Vinte e quatro (25,8%) crianças apresentaram perda de DMO maior que 10% com relação à DMO adequada para a idade e sexo. A DMO mostrou-se significantemente menor em relação ao grupo controle nos grupos: com hipermobilidade (independente da presença de dor), com dor (independente da presença de hipermobilidade), com hipermobilidade e sem dor e sem hipermobilidade e com dor.Conclusão: a DMO pode estar diminuída em crianças com hipermobilidade (independente da presença de dor músculo-esquelética) e em crianças com dor (independente da presença de hipermobilidade) em relação aos controles.J Pediatr (Rio J) 2002;78(6):523-8: densidade mineral óssea, hipermobilidade articular, dor músculo-esquelética. The aim of the study was to test whether an association exists between asthenic somatotype, as measured by the Quetelet index, and panic and or agoraphobia. Ninety‐nine cases with panic and/or agoraphobia diagnosed at the psychiatric outpatient clinic of a general teaching hospital were compared to 99 controls with psychiatric conditions other than panic and agoraphobia, and to 64 medical controls, matched by age and gender. No differences in weight, height and raw Quetelet and ponderal indexes were found between panic cases and controls. However, 33.3% of cases, but only 19.2% of psychiatric controls and 18.7% of medical controls, were in the asthenic group as defined by the lower quartile of the Quetelet index (age and sex adjusted odds ratio = 2.33, 95% C.I.: 1.21–4.11). In the entire sample, the asthenic somatotype was associated with lower age and with the presence of hypermobility of joints. Since both panic disorder and mitral valve prolapse may be associated with the joint hypermobility syndrome (FHS), an inherited disorder of collagen synthesis, it is suggested that the relationship found between panic and asthenic somatotype might be mediated through FHS.
