This case report discusses a 77-year-old female patient who presented to an outpatient clinic with urinary symptoms and recurrent UTIs. Imaging revealed a foreign body, which was later confirmed as a retained intrauterine device (IUD) that had caused a vesicouterine fistula (VUF). The patient had a medical history of cervical cancer treated with radiation therapy, during which her IUD's string could not be located, leading to the decision to proceed with radiation therapy without removing the IUD. The patient opted to manage her condition medically rather than undergo surgical removal due to concerns about worsening the vesicouterine fistula. This case highlights the potential risks and complications of retained IUDs, and the importance of careful consideration and communication among clinical teams and patients when managing these situations.

Background Among all urogenital fistulas Vesicouterine fistula (VUF) is the rarest. The most common cause of VUFs is a previous cesarean section. We report a patient with a unique presentation of VUF, which is a protruded amniotic sac through urethra. Case presentation A 29-year-old woman in her fifth pregnancy presented to the emergency department at 24 weeks gestation with an abdominal pain and a sac protruding from vulva during urination thinking she was aborting. The pelvic examination revealed a thin-walled fluid-filled 5 cm × 5 cm- sized clear serous sac protruding from her urethra with no connection of the cervix or bleeding. Gynecologic examination showed partial cervical canal dilation without bleeding or leakage of amniotic fluid. Based on the clinical and ultrasound findings, we suspected a uterine and bladder rupture with the amniotic sac protruding from the urethra, therefor an emergency cesarean section was done. During the procedure, we addressed the fistula and The patient was discharged after a two-day recovery period. Conclusions Vesicouterine fistulae could interfere with many other disorders and should be considered as a differential diagnosis in every woman with a protruding amniotic sac.

A G4P4 woman in her 30s with a type II vesicouterine fistula, as defined by the Jozwik classification system, presented with symptoms of menouria, vaginal menses and urinary incontinence 8 years after caesarean delivery, the time of probable origination of the fistula tract. Transvaginal ultrasound identified a fistula tract communicating between the bladder and uterus, a rare finding that many years remote from caesarean delivery. Traditional surgical technique includes laparoscopic, abdominal and endoscopic methods of repair, sometimes using a transvesical approach. Transvesical repair can be associated with subsequent inpatient hospital stays and prolonged catheterisation. Our technique proposes a transvaginal surgical approach as an outpatient procedure with decreased operating time (40 min), postoperative pain and catheterisation requirement. It is the authors’ belief that a transvaginal approach is less invasive and allows for better preservation of the uterus for future pregnancies and vaginal deliveries, as desired by the patient.

Abstract: Here, we report a 41‐year‐old female with type II VUF after hysteromyomectomy.We report the diagnosis of VUF by imaging method, and provide a feasible treatment for this complication after pelvic surgery.
