A Case Report on Parvovirus B19 Associated Myositis

Abstract: Introduction. Whilst there are reports of viral myopathies affecting children and the immunocompromised, infective myositis is a relatively rare inflammatory myopathy in adults. The clinical spectrum can range from benign myalgias to more serious complications in certain risk groups. Case Presentation. We present two cases of myositis as a result of parvovirus B19 infection. Conclusion. Viral myositis and parvovirus B19 associated myositis should be considered in adults presenting with significant myalgia.

“…Gangrene of stomach or intestine owing to non-occlusive bowel infarction (NOBI) has an unknown aetiology 8 9 10 11 with no further information after our report 5 , and same is the status for amegakaryocytic thrombocytopenia due to B19 infection. However, there are couple of reports and a review article on myositis induced by B19 that supports our finding 12 13 14 . Results of previously reported cases of NOBI had shown B19 genome in serum samples of all eight cases (in three cases resected bowel tissues also) besides anti-B19 antibodies and thrombus in gastric vessels 5 .…”

“…Other syndromes for which a causative role for B19V is unclear include generalized edema (963)(964)(965)(966), myositis (967), the onset of diabetes (968) and inflammatory bowel disease (969), uveitis (970), retinal detachment (971), retinal pigment epitheliopathy (972), Rosai-Dorfman histiocytic proliferation (973), laryngitis (974), inner ear disease (975), and testicular tumors (976). B19V DNA persistence in the thyroid after previous infection may account for associations with thyroid cancer (191) and autoimmune thyroiditis (977).…”

“…There are several case reports of parvovirus B19 infection causing myositis in adults, but, in one case series, muscle biopsies in patients with myositis and parvovirus infection did not reveal the presence of B19 DNA, suggesting that parvovirus is not directly responsible for causing the myositis [6, 27]. Nevertheless, there may be an indirect immunologic mechanism involved, as parvovirus infection has been thought to precipitate several autoimmune disorders, most famously rheumatoid arthritis and systemic lupus erythematosis.…”
