Introduction: Meckel's diverticulum or ileal diverticulum is one of the common congenital anomalies of the digestive system. It may not cause any problems in many, but may form serious life threatening complications in a few. Materials and Methods: We conducted a histo-morphological study of a giant Meckel's diverticulum found during cadaveric dissections of a South Indian adult male cadaver. The diverticulum was 7.5 cm long and had a circumference of 1.5 cm. Results: Gross anatomical and histological observations revealed healthy nature of the diverticulum without any inflammation. However, the mucosa had gastric type of glands with plenty of parietal cells. This incidence of Meckel's diverticulum was noted in only one cadaver among more than 100 cadavers dissected in the past twenty years. Conclusion: Knowledge of its morphological features may be very useful to surgeons, radiologists and gastroenterologists. We discuss the clinical importance of the diverticulum and review the relevant literature in this manuscript.

Background: Meckel's diverticulum is relatively rare in the general population. Generally, symptoms are extremely varied and prior diagnosis is difficult because of the abnormality being an occasional finding in laparotomies. This report describes the unusual possibility of basement of diverticulum rupture, even after low-impact abdominal trauma. The case is unusual because of the low frequency of small bowel Injury in abdominal contusions, except in trauma with high kinetic energy, such as high-speed car crashes, and the unexpected finding of a ruptured base of diverticulum. Usually, the lesions occur at relatively fixed structures, specifically in the angle of Treitz or ileocecal region, after undergoing stretching.

Цель. Представить клиническое наблюдение редкого осложнения дивертикула Меккеля (ДМ). Основное содержание. Заболевания, обусловленные наличием ДМ, служат одной из наиболее распространенных причин «острого живота» у детей: до 60 % случаев их манифестации приходится на возраст до 10 лет. Чаще всего проявлениями ДМ являются такие осложнения, как кишечная непроходимость, кровотечение, дивертикулит, грыжа Литтре. 9-летняя девочка с абдоминальной болью и рвотой, появившимися несколько часов назад, была госпитализирована в детское хирургическое отделение. Спустя несколько часов ребенок был оперирован: обнаружена странгуляционная кишечная непроходимость на мезодивертикулярной связке, соединяющей дивертикул Меккеля с корнем брыжейки тонкой кишки. Была выполнена резекция дивертикула и устранение непроходимости кишки. Ребенок выписан с выздоровлением. Заключение. Заворот кишечника вокруг мезодивертикулярной связки является одним из наиболее редких осложнений ДМ. При запоздалой диагностике данная ситуация опасна развитием грозных осложнений в виде некроза и перфорации кишечника, что делает дивертикул Меккеля одним из опасных состояний периода детства. Aim.To present of a clinical case of a rare complication of Meckel's diverticulum (MD). General fi ndings. Diseases caused by the presence of MD present one of the most common causes of an "acute abdomen" in children: up to 60% of cases of their manifestation occur in children under 10 years of age. The most common MD manifestations involve complications, such as intestinal obstruction, bleeding, diverticulitis and Litre hernia. A 9-year-old girl with abdominal pain and vomiting having appeared several hours before hospitalization was admitted to a paediatric surgical department. A few hours later, the child was operated. In the course of surgery, a strangulated intestinal obstruction was found on the mesodiverticular ligament connecting Meckel's diverticulum with the root of the small intestine mesentery. As a result, a diverticulum resection and bowel obstruction were performed. The child was discharged with recovery.Conclusions. An inversion of the intestine around the mesodiverticular ligament appears to be one of the most rare complications of MD. In this case, the delayed diagnosis results in the development of formidable complications in the form of necrosis and intestinal perforation, which makes Meckel's diverticulum one of the most dangerous conditions in children.Шидаков Ислам Хусеинович -врач -детский хирург детского хирургического отделения РГБЛПУ «Республиканская многопрофильная детская больница».
