Tinea nigra is an uncommon superficial dermatomycosis precipitated by Hortaea werneckii, a halophilic and halothermic yeast-like fungus capable of producing a melanin-like substance. This pathogen infiltrates the stratum corneum in the setting of microtrauma and produces an asymptomatic brown to black macule or patch that appears similarly to melanocytic nevi or melanoma. We present a case of a 52-year-old woman who presented to clinic several months after developing a painless, nonpruritic dark brown patch in her left foot inside the fourth toe web. The coloration and location of this lesion would typically prompt biopsy; however, Wood's lamp examination and potassium hydroxide (KOH) preparation were pursued first and demonstrated evidence of infection by H. werneckii. The patient was treated with topical clotrimazole cream and miconazole powder for one month, and her lesions cleared completely. Her lesions did not recur at her three-month follow-up appointment.

Die Krankheit kommt haufiger in den tropischen und subtropischen Landern vor (7, 9, 23, 31,44), sie ist jedoch auch in Landern mit gema5igter (15,29,32,43) und kiihler Temperatur (20, 51) beobachtet worden.Die Mykose kann n i t Ausnahme des Gesichtes (8, 40,45) alle Hautpartien befallen. Allerdings scheinen bei den verschiedenen Lokalisationen geographische Faktoren von Bedeutung zu sein.…”

Zusammenfassung Es wird über den ersten Fall von Tinea nigra palmaris in Deutschland berichtet. Klinik, Histopathologie, Mykologie, Pathogenese und Therapie werden besprochen. Diese harmlose, oberfälchliche Pilzkrankheit kommt vorwiegend in den tropischen und subtropischen LäLndern vor. Sie wird durch Cladosporium wernedci und C. mansoni verursacht. Das Leiden fängt mit einem stecknadelkopfgroßen, braunen Fleck an, der sich langsam vergörßert. Der Erreger befindet sich vorwiegend in der obersten Hornschicht und verursacht außer einer mäßigen Hyperkeratose keine nennenswerten Gewebsveränderungen. Auf Sabouraud'schem Nährboden wachsen bei 28° C nach einer Woche kleine schwarze Kolonien, die mikroskopisch aus ovalen Zellen mit zentralen Septen sowie vom Akrotheka‐ und Hormodendrum‐Sporulationstyp bestehen. Eine Hautscähdigung ist wahrscheinlich eine Voraussetzung für die volle Entwicklun der Tinea nigra. Die erfolgreiche Behandlung wurde mit 5‐Fluorocytosin in 10%iger Salbenform durchgeführt.
