A Case of Sheehan's Syndrome that Manifested as Bilateral Ptosis

Abstract: Hypothyroidism can cause a variety of signs and symptoms of the neuromuscular system. However, ptosis in a patient with hypothyroidism is very rare. We report here on a case of central hypothyroidism that was due to Sheehan's syndrome and it manifested as bilateral ptosis in a 51-yr-old woman. She complained of exertional dyspnea and weakness. About 25-yr ago, she had a history of severe postpartum vaginal bleeding. The laboratory studies demonstrated hypopituitarism with secondary hypothyroidism. The ptosis w… Show more

“…Sheehan's syndrome or adenohypophyseal ischemic necrosis occurs following hypoperfusion as a result of ischemic pituitary necrosis due to severe postpartum hemorrhage [7][8][9][10]. Prolactinmediated anterior pituitary hypertrophy during pregnancy seems to render the gland susceptible to ischemic necrosis in the presence of hypovolemic shock, and it is most frequently associated with hemorrhagic shock in the latter stages of labor.…”

“…Sheehan syndrome exhibits partial or complete hypopituitarism caused by ischemic pituitary necrosis due to obstetrical shock, accompanied by DIC following severe postpartum hemorrhage [4]. However, it is difficult to predict the possible onset of Sheehan syndrome in patients suffered from massive hemorrhage during delivery, while most cases had mild hypopituitarism and failed to be diagnosed for a long time [5].…”

“…Sheehan's syndrome occurs as a result of ischemic pituitary necrosis due to severe postpartum hemorrhage [ 7 – 10 ]. Only a small proportion of patients with Sheehan's syndrome may have a sudden onset of severe hypopituitarism immediately after delivery, whereas most patients have mild illness and they have not been diagnosed for a long time so that they are not treated appropriately [ 4 ].…”
