Background and Objectives:The submandibular lipomas are relatively rare entity. The reported age of the patients were from birth to 84 years old, and average being 55.7 years. These are mainly asymptomatic and mostly cause aesthetic concerns. Deep lipomas, especially in the head and neck , are not commonly reported. To our knowledge there's not even a single publication regarding the lipoma in the submandibular region in our mid western region as well as in our country. Presentation of case:A 72 years old female presented with complaints of painless progressive neck swelling in the right submandibular region for about 2 years. Clinically, it was thought to be submandibular gland neoplasm/ lymphadenopathy. Ultrasonography (USG) and fine-needle aspiration cytology (FNAC) were done. It was negative for malignancy. She underwent excision of the mass under general anesthesia without postoperative complications. The histopathology confirmed it to be lipoma. Discussion: Lipoma is a common benign mesenchymal tumour that is found almost anywhere in the body. However, a large lipoma is relatively rare in the submandibular region. On clinical doubt, one can rely on USG / MRI (magnetic resonance imaging) and FNAC to rule out diagnostic dilemma. The treatment of choice is simply excision. Conclusion:It's a rare entity and may be the first report of a lipoma in the submandibular region in an elderly people. One should rule out its possibility while going through the submandibular region diseases.

Lipoma involving multiple fascial spaces is extremely rare and poses a challenge to surgeons using less invasive procedures. Although blunt dissection using a finger is often used in excisional surgeries as a supplementary maneuver, few cases have been described using the bimanual technique for the removal of these extensive lesions. Herein, the authors present a large lipoma of the oral floor extending to the unilateral submandibular and parapharyngeal spaces, which was excised only via a submandibular approach using the “push–pull down” maneuver, blunt finger dissection of the tumor with counter-pushing on the overlying mucosa to the submandibular direction facilitated tumor separation. The tumor was consequently pulled down and removed without an additional intraoral incision. Based on accurate preoperative examinations, this maneuver, provides a less invasive surgery for well-encapsulated benign tumors involving multiple fascial spaces.

Os lipomas são definidos como neoplasias benignas cuja sua origem é mesenquimal, apresentam crescimento lento e é bastante incomum encontrá-los na cavidade bucal, apesar de ser frequentemente encontrado em outras regiões do corpo, principalmente nas extremidades e em locais onde há maior presença de adipócitos, tendo em vista que, histologicamente, esse tumor é constituído por adipócitos maduros. Clinicamente, é caracterizado como lesões nodulares que podem ser sésseis ou pediculares e é diagnosticado, geralmente entre a quarta e sexta décadas de vida. O presente estudo irá interrelacionar o relato de um caso clínico raro de lipoma de grandes proporções em região submandibular em paciente masculino com a literatura a partir de uma revisão narrativa/crítica através do acesso às principais bases de dados, a saber; PubMed (Medline), Web Of Science, Scopus e Lilacs utilizando os termos do vocabulário controlado do DeCS.
