Leiomyoma is the most common oesophageal tumour and is symptomatic in about half of the patients. Dysphagia is the most common symptom. A 41-year-old woman presented with recent onset of severe orthostatic hypertension. During the cardiological work-up, a mediastinal mass was found at transthoracic echocardiogram. Further investigation (upper gastrointestinal endoscopy, endoscopic ultrasonography, CT scan and cardiac MRI) confirmed the diagnosis of a large oesophageal submucosal mass compressing the supradiaphragmatic inferior vena cava. The mass was resected through a minimally invasive right thoracoscopic approach with complete relief of symptoms and compression on the vena cava.

Oesophageal leiomyoma is a rare benign tumour of the oesophagus, which does not cause clinical symptoms in more than half of cases. Below we present the case of a symptomless oesophageal tumour. Due to the unequivocal result of imaging examinations and no histopathological diagnosis the patient was qualified for thoracotomy. During the operation the oesophageal tumour was suspected of passing through the hiatus into the abdominal cavity. The specimen revealed a neoplasm of mesenchymal origin without distinct traits of malignancy. The patient was admitted to the Surgical Department, where she was qualified for surgery, which was carried out by a team of surgeons and thoracic surgeons. The encapsulated tumour and oesophagus were resected. The histopathological and immunohistochemical examinations corres ponded to leiomyoma oesophagi. The lesion was radically resected. The size of the lesion was 22 × 14 × 13 cm. Three months after the surgery no traits of relapse were found.

Klíčová slova: Absces Ezofagus Komprese levé síně Mediastinum Srdeční metastázy Srdeční selhání SOUHRNLevá síň je relativně tenkostěnný srdeční oddíl s nízkým intraluminálním tlakem, který je z anatomického hlediska v úzkém kontaktu se strukturami v zadní části dolního mediastina, a to především s jícnem a descendentní aortou. Tyto charakteristické vlastnosti ji dělají náchylnější k zevní kompresi patolog ickými strukturami souvisejícími s gastrointestinálním traktem, mediastinem, plícemi, perikardem a aortou. Podle stupně útlaku levé síně může být pacient asymptomatický nebo se mohou objevit známky kardiální insufi cience. V naší kasuistice popisujeme letálně probíhající útlak levé síně chronickým abscesem mediastina vzniklým rupturou zadní části jícnu při ulcerující ezofagitidě. Dle našeho zjištění jde o první podobný publikovaný případ, neboť ohraničený průběh mediastinitidy se vznikem abscesu je extrémně raritní.
