A Case of Cutaneous Malignant Fibrous Histiocytoma

Abstract: A 54-year-old Japanese man with cutaneous malignant fibrous histiocytoma on the back is reported. He not only had a past history of thyroid cancer 1 year prior to the onset of the skin tumor, but also had simultaneous bladder cancer. Despite the early, wide resection, the prognosis was rapid and progressive. Histologically, the primary lesion of the skin tumor was difficult to differentiate from dermatofibrosarcoma protuberans; however, the recurrent and the metastatic lesions changed in appearance.

“…A correct interpretation is important, since the prognostic and treatment vary significantly among those entities. 1,4 Ample excision is recommended, due to the high degree of local recurrence (approximately 44%) and metastasis, most commonly in the lungs (approximately 42% of cases). 2,5,7,8 The presence of metastasis is usually associated with a poor prognostic.…”

“…Most of them, however, arise from the retroperitoneal space [4, 911]. IMFH presenting as an ulcerative cutaneous lesion is extremely rare [8]. An elevated WBC is universally associated with this sarcoma; however, a leukemoid reaction with a leukocyte count above 50 K/mm 3 is extraordinary [7, 12].…”

“…IMFH associated with leukemoid reaction is rare and its presentation as a destructive cutaneous lesion is also unusual [8]. Here we discuss a case of an aggressive inflammatory MFH of dermal/epidermal origin located on the left upper extremity presenting with a severe leukemoid reaction.…”
