A Case of Cardiac Hydatid Cyst Localized in the Lungs Bilaterally and on Anterior Wall of Right Ventricle

Abstract: A 23-year-old man had been coughing with hemoptysis, and had received tuberculostatic medication for four months without any benefit. Upon referral, two-dimensional echocardiography showed a cystic mass located in the anterior right-ventricular wall, without any protrusion into the ventricular cavity. CT examination revealed three cysts in the lung fields bilaterally, additionally a multilocular cystic image in the right-ventricular wall was observed. All components of hydatid cysts in the heart and lungs were… Show more

“…1,21,43,44 It has been reported that pulmonary hydatid cysts have a predilection for the right lower lobe of the lung, whereas bilateral involvement occurs in 20% of cases and multiple cysts in 30%. 1,43,44 Pomelov et al 45 reported that concurrent involvement of the liver and lungs is seen in approximately 6% of all patients with hydatid cysts in the thoracic and abdominal organs. It is interesting to note that because of the compressibility of the lungs, it is the only organ in which hydatid cysts can grow so large, especially in childhood and adolescence.…”

“…1,47 Complications of pulmonary hydatidosis are rupture of the cyst into the parenchyma or pleural cavity, causing consolidation surrounding the cyst. 1,44 The safety and efficacy of percutaneous treatment for pulmonary hydatid cysts was first reported by Akhan et al, 48 whereby the authors proposed the method as an alternative to surgery in selected patients who failed medical therapy.…”

“…From there, they may reach the left heart and coronary arteries through the foramen ovale or the pulmonary capillary bed (2). Cardiac echinococcosis should be suspected in any patient from an endemic area with a history of close contact with dogs or sheep who has eosinophilia or radiographic evidence of cystic disease in other organs (12). Serologic tests alone cannot be used to definitively establish or exclude echinococcosis (13).…”

“…Atrial cysthydatid is a very rare form. 3 We presented a case with multiple cardiac cysthydatid originating from interatrial septum at right atrium and localizing on epicardial surface of LV apex and having coronary artery disease, which we treated surgically in light of the literature.…”
