A Case of Bullous Transient Acantholytic Dermatosis

Abstract: We report a case of a bullous variant of transient acantholytic dermatosis in a 59-year-old female. Each bullous lesion lasted several weeks and healed without scarring. The lesions were migratory and recurrent without a cleared period. Histopathologic examination revealed an intraepidermal vesicle low in the epidermis. The lesions cleared after 2 months of isotretinoin therapy.

“…Such cases should be classified into the broad spectrum of acantholytic dyskeratotic diseases of Ackerman [10,11]. Similar changes reported as persistent transient acantholytic disease [6, or atypical Darier's disease [ 12,201 andor dyskeratomatosis verruciformis [I41 also belong to the group of acantholytic dyskeratosis.…”

“…In 1970, Grover [3] coined the term transient acantholytic dermatosis (TAD) for papulo-vesicular eruption, localized preferentially on the trunk, with characteristic histological pattern of pronounced acantholysis and scarce, if any, dyskeratosis [3,4] except for Darier's type variant with pronounced dyskeratosis [4]. Later there were reported various other forms of TAD: bullous [5,6] and plaque [7] variants, and persistent acantholytic dermatosis keratosis (FAD). According to number, size and duration of the lesions, they were classified into solitary (papular, nodular), multiple peTistent (Darier's disease, linear, zosteriform) and evanescent (transient acantholytic dermatosis).…”

“…Antihistamines can decrease itching (2, 13). In therapy-re sistant cases, oral retinoids such as etretinate (31) and isotretinoin (26,(32)(33)(34) can be used. Vitamin A (13,35) and clindamycin (36) were successfully given in some cases of TAD.…”

“…Papular acantholytic dyskeratosis with oral isotretinoin acquired good results [157]. Yoo (1994) reported a case of a bullous variant of transient acantholytic dermatosis in a 59-year-old female. Each bullous lesion lasted several weeks and healed without scarring.…”
