A Case of Acquired Deficiency of Pituitary GH, PRL and TSH, Associated with Type 1 Diabetes Mellitus

Abstract: Abstract.A 75-year-old male showed combined anterior pituitary hormone deficiency (CPHD). Basal and TRHstimulated PRL levels were undetectable. Basal and GRH-stimulated GH levels were very low, and could barely be measured by means of an ultrasensitive enzyme immunoassay. In addition, basal TSH levels were under the normal limit, and TRH-stimulated TSH secretions were impaired. On the other hand, the secretions of ACTH, LH and FSH remained intact. There was no mutation of Pit-1 gene in this patient, and immuno… Show more

“…In the present case, HLA typing detected DR9, which has been reported to be associated with increased susceptibility to both type 1 diabetes and Hashimoto's thyroiditis in Japanese autoimmune polyglandular syndrome patients (11). Moreover, we also identified HLA A24 and Cw3, which are frequently detected in individuals with lymphocytic hypophysitis (12,13). The clinical features, endocrine abnormalities and HLA status in the current patient may also be in accordance with the diagnosis of lymphocytic hypophysitis.…”

“…The role of pituitary hormone is unclear. A case of acquired deficiency of pituitary GH, PRL, and TSH associated with T1D mellitus was reported [ 81 ]. But basal TSH concentrations were unchanged with or without metabolic control in patients [ 82 ].…”

“…A 75-year-old man was admitted to the Jikei University Hospital for glycemic control of diabetes mellitus (DM) (12). Because he was anti-GAD antibody-positive, he was diagnosed with slowly progressive IDDM (SPIDDM) and thereafter treated with insulin.…”
