A Case‐Control Study to Investigate Serological Correlates of Clinical Failure of 23‐Valent Pneumococcal Polysaccharide Vaccine in HIV‐1–Infected Ugandan Adults

Abstract: We have investigated the association between the concentration of anti-polysaccharide pneumococcal capsule-specific (anti-PS) immunoglobulin G and the killing activity, in serum, in invasive pneumococcal disease (IPD) events and response to 23-valent polysaccharide vaccine in human immunodeficiency virus (HIV)-infected Ugandans. Case patients with IPD had lower concentrations of anti-PS IgG before and after vaccination and before the IPD event ( for 5 [i.e., 4, 9V, P ! .01 14, 18C, and 19F] of 6 serotypes asse… Show more

“…Previous studies have suggested that a compromised memory B-cell response and/or low specific antibody titers may increase the risk of infections for the HIV-1 carrier 10,[13][14][15] and for children born to HIV-1-infected mothers. [16][17][18][19] To further dissect the relationship between loss of memory B cells, reduction of specific antibody levels, and disease progression, we studied subjects at different stage of disease (ie, PHI, CHI, and long-term nonprogressor [LTNP]) and also evaluated the effects of ART.…”

“…The immunization increased the antibody levels in adults (18), pregnant women (19,20), healthy older people (21) and HIV-infected individuals (11,17,22). A 7-valent conjugate pneumococcal vaccine followed by a boost with the 23-valent polysaccharide vaccine resulted in a twofold increase in the levels of IgG specific for the serotype antigens 1, 6B, 9V, and 14 in 66% of HIV-infected adults (23), indicating that the response to vaccination against pneumococcal infection is an appropriate strategy to assess the level and quality of immune function.…”

“…L'immunogénicité faible de ce vaccin est probablement la cause de son efficacité inconstante [34]. Les études immunologiques réalisées chez les patients infectés par le VIH ont toujours retrouvé une réponse anticorps significative mais sub-optimale, le taux d'anticorps mesuré étant plus bas que celui mesuré après une même vaccination chez le patient immunocompétent [35,36].…”
