A case-control study of major congenital heart defects in Sweden ? 1981?1986

Abstract: This paper reports the results of a case-control study of major congenital heart defects (CHD) in Sweden. During the period 1981-1986, 1,324 such cases were identified and 2,648 controls were selected. Some common maternal characteristics and exposures were studied using information from prospectively collected data. Possible associations with CHD were found for previous perinatal death, maternal diabetes, epilepsy, hydramnios and disproportion between fetus and pelvis. More specific associations were observed… Show more

“…Some recent epidemiological studies have clarified that maternal lifestyles, such as smoking or drinking habits and passive smoking during pregnancy, provoke some congenital anomalies (Jones and Smith, 1973;Himmelberger et al, 1978;Ericson et al, 1979;Kuroki, 1988;Jacobson et al, 1993;Yuan et al, 1994), although there are negative reports (Borlee et al, 1978;Marbury et al, 1983;Pradat, 1992). However, to our knowledge, an association of anal atresia with smoking and/or drinking during pregnancy has not been previously reported.…”

“…82 Because several therapy-related factors could account for this increased risk, including direct teratogenic effects of anticonvulsant drug therapy and an indirect effect of the drugs by interfering with folate metabolism, it has been difficult to determine whether maternal seizures are independently associated with an increased risk of heart defects.…”

“…Epilepsy and anticonvulsant medications also have been studied as possible risk factors for CVM. Several epidemiological studies identi®ed associations between CVM and epilepsy [Dansky and Finnell, 1991;Pradat, 1992], but an unresolved question is whether the underlying risk factor is some aspect of the disease itself or of speci®c anticonvulsant medications, many of which are strongly implicated as teratogens [Hanson et al, 1976;Bossi, 1983;Zierler and Rothman, 1985;Bracken, 1986]. No speci®c pattern of heart defects seems to be closely related to these factors, with reported associations involving CVM as a whole as well as the common arterial trunk, transposed great arteries, and septal defects.…”
