ÖzBruselloz, tüm dünyada yaygın zoonotik bir hastalıktır ve ülkemizde de birçok bölgede endemik olarak görülmektedir. Klinik seyri sırasında sık olarak retiküloendotelyal sistem organlarının tutulumu gözlenen hastalığın başlıca semptom ve bulguları yüksek ateş, kas-eklem ağrıları, halsizlik, iştahsızlık ve yorgunluktur. Cilt tutulumu nadir görülen bir bulgu olarak karşımıza çıkmaktadır. Bu nedenle bu yazıda ateş yüksekliği ve tüm vücutta yaygın makülopapüler döküntü ile başvuran 12 yaşında bir erkek hasta sunulmuştur. Involvement of reticuloendothelial system is seen frequently during the clinical course. The most common symptomps and signs are fever, joint pain, myalgia, back pain, anorexia and fatigue. Skin involvement is rarely seen. Thus in this paper we report a 12-year-old boy presenting with fever and diffuse maculopapular rash. Esas olararak hayvanlarda hastalık yapan brusella türlerinin insanlara bulaşı enfekte canlı veya ölü hayvanlarla direk temas, pastörize edilmemiş süt ve süt ürünlerinin tüketimi, enfekte aerozollerin inhalasyonu ve konjunktivaya inokü-lasyon ile gerçekleşir [1][2][3]. Hastalığa sebep olan brusella türleri küçük, hareketsiz, Gram negatif kokobasillerdir [1]. Klinik seyri sırasında sık olarak retiküloendotelyal sistem organlarının tutulumu gözlenen hastalığın başlıca semptom ve bulguları yüksek ateş, kas-eklem ağrıları, halsizlik, iştahsızlık ve yorgunluktur [1,2]. Cilt tutulumu nadir görü-len bir bulgu olmakla beraber brusellozun birçok hastalığı taklit edebileceği akılda tutulmalıdır. Hastalık seyri sıra-sında hematolojik, kardiyopulmoner, genitoüriner ve santral sinir sistemi gibi tutulumları görülebilmekte ve bunların sonucunda ciddi morbidite olabilmektedir. Zamanında ve etkin tedavi edilmemesi kronikleşme, birçok sistemi ilgilendirebilecek komplikasyonlar ve rölapslara neden olabilmektedir [1][2][3]. Bu nedenle erken tanı ve tedavi komplikasyonların önlenmesi açısından önemlidir. Brusellozda yaklaşık vakaların %5 kadarında döküntü, papül, ülser, abse, eritema nodozum, peteşi, purpura ve vaskülit gibi cilt lezyonları görülebilmektedir [1][2][3][4][5]. Brucellozda her ne kadar cilt bulguları nadir olarak görülse de erken tanı ve tedavi ile, gelişebilecek ciddi morbiditelerin önlenebi-lir olmasından dolayı özellikle ülkemizde ateş yüksekliği ve döküntülü hastaya yaklaşırken bruselloz da olabileceği unutulmamalıdır. Brucellosis is a zoonotic infectious disease of worldwide distribution and is still endemic in some developing countries. Brucellosis is a systemic disease in which any organ or system of the body can be involved. The most commonly affected systems are the locomotor, gastrointestinal, genitourinary, haematological, cardiovascular, respiratory, and central nervous systems. Cutaneous manifestations are rare in brucellosis and the lesions are not specific to the disease. Skin involvement is reported to range between 0.4% and 17.0% of the patients with brucellosis. The cutaneous manifestations in brucellosis are seen because of the direct inoculation of bacteria into the skin, hypersensitivity phenomena, deposition of immune complexes in the skin, and invasion of the skin via a haematogenous route of spread of the micro-organism. Papulonodular and maculopapular eruptions, and erythema nodosum-like lesions are the most frequently encountered cutaneous lesions in brucellosis. Brucellosis should be kept in mind by clinicians in the diagnosis of patients with complaints of fever and eruptions, especially in endemic regions.
