Purpose This study aims to assess the testing methods used to detect fungal pathogens as well as the pathogenic profiles and drug susceptibility among fungal isolates from ocular sources collected in a tertiary eye hospital. Methods The laboratory test records of patients with suspected ocular fungal infection from January 2010 to December 2019 at a tertiary eye care center in southern China were retrospectively evaluated. The isolated pathogens identified by smear examination and culture combined with microscopy were analyzed. For 2017–2019 fungal isolates, their antifungal susceptibilities to four antifungal agents were evaluated. Results A total of 22,233 specimens (16,315 for culture and 5918 for direct smear examination) from 16,209 individuals were assessed, and 3317 specimens (2169 for culture and 1148 for smear examination) from 2395 patients had confirmed fungal infection. The mean age of patients was 50.6±14.2 years and 63.4% were male. The positive rate of smear examination was 19.4% and significantly higher than 13.3% of fungal culture (p< 0.001). Of 5812 patients who were simultaneously examined for culture and smear staining, 897 (15.43%) had positive findings for fungi. Among the 2420 positive findings, 2138 (88.3%) were from the cornea, and 133 (5.5%) were from intraocular samples. Fusarium spp. (40.5%) and Aspergillus spp. (22.0%) were the major fungal genera among the molds, whereas Candida spp. (4.6%) constituted the major yeast. Fusarium spp. had a lower antifungal susceptibility than Aspergillus spp. against voriconazole and amphotericin. And yeast spp. had the highest susceptibility to caspofungin. Conclusion This study provided a 10-year assessment of fungi in ocular infections in southern China, giving support to epidemiological understanding and guiding empiric antimicrobial therapy.

Background The Republic of Congo (RoC) is characterised by a high prevalence of tuberculosis and HIV/AIDS, which largely drive the epidemiology of serious fungal infections. Objective We aimed to estimate the current burden of serious fungal infections in RoC. Material and Methods Using local, regional or global data and estimates of population and at‐risk population groups, deterministic modelling was employed to estimate national incidence or prevalence of the most serious fungal infections. Results Our study revealed that about 5.4% of the Congolese population (283 450) suffer from serious fungal infections yearly. The incidence of cryptococcal meningitis, Pneumocystis jirovecii pneumonia and disseminated histoplasmosis in AIDS patients was estimated at 560, 830 and 120 cases per year. Oral and oesophageal candidiasis collectively affects 12 320 HIV‐infected patients. Chronic pulmonary aspergillosis, 67% post‐tuberculosis, probably has a prevalence of 3420. Fungal asthma (allergic bronchopulmonary aspergillosis and severe asthma with fungal sensitisation) probably has a prevalence of 3640 and 4800, although some overlap due to disease definition is likely. The estimated prevalence of recurrent vulvovaginal candidiasis and tinea capitis is 85 440 and 178 400 respectively. Mostly related to agricultural activity, fungal keratitis affects an estimated 700 Congolese yearly. Conclusion These data underline the urgent need for an intensified awareness towards Congolese physicians to fungal infections and for increased efforts to improve diagnosis and management of fungal infections in the RoC.

Purpose: The aim of this study is to investigate the epidemiological and aetiological pattern of fungal corneal ulcers treated in Hospital Bintulu, Sarawak.Study design: Cross-sectional study.Methods: This study is based on the data collected from clinical records of patients with culture-positive fungal corneal ulcer who presented to Hospital Bintulu from 2016 to 2020. Data was analysed using descriptive statistical methods.Results: A total of 13 patients had fungal corneal ulcer. Males (84.6%) were more commonly affected compared to females (15.4%). The age of the patients ranged from 19 to 67 years. The commonest age group involved was 21–40 years, constituting more than half of the patients (53.8%). The mean age of the patients was 38 years old. Fusarium sp. was isolated in eight of them (61.5%), all of which had a history of palm oil dust insertion into the eye prior to presentation. Other fungal pathogens found include Phellinus noxius sp. in two cases (15.4%) as well as Ascomyces sp., Nectriaceae sp., and Colletotrichum trunchatum sp. in one case each (7.6%). Hypopyon was seen in eight cases (61.5%), where six cases were caused by Fusarium sp. while Ascomyces sp. and Nectriaceae sp. contributed one case each. Mixed fungal and bacterial infection was seen in two cases (15.4%).Conclusion: This study showed that Fusarium sp. is the commonest pathogen causing fungal corneal ulcer and affects predominantly young male plantation workers. The establishment of a proper framework to educate as well as strict enforcement of occupational safety may reduce the incidence of this disease in the population.
