A 10-Year Comparison of Endogenous Endophthalmitis Outcomes: An East Asian Experience With Klebsiella Pneumoniae Infection

Abstract: The authors' experience with endogenous endophthalmitis has confirmed the findings of analogous East Asian studies, with liver abscess as the major source of infection and K. pneumoniae as the causative organism. Superior outcome may be achieved with more comprehensive and collaborative management, ideally through the combined efforts of multiple medical subspecialties.

“…7 Endophthalmitis caused by Klebsiella species has been recognized as an increasingly prevalent cause of endogenous endophthalmitis in Asia, accounting for 54 to 61 percent of cases in selected series. 5,[8][9][10][11] Reported risk factors for endogenous endophthalmitis caused by Klebsiella species include liver abscesses, diabetes mellitus, and systemic immunocompromise. 5,8,12,13 In contrast, endophthalmitis caused by Klebsiella, whether endogenous and non-endogenous, is uncommon in the USA, described only in a small number of isolated case reports and series.…”

“…Therefore, serotype K1 Klebsiella pneumonia liver abscesses are no longer fatal but causes catastrophic disabilitie due to irreversible ocular and central nervous system complications (7). Chen Y-J et al reported that patients with Klebsiella pneumonia endogenous endophthalmitis whose visual acuity before the initiation of treatment included counting fingers or better achieved good visual outcomes (14). It is important that antibiotic therapy be initiated before the patient's visual acuity worsens.…”

“…Okada et al 25 , Wong et al 29 and Jackson et al 30 stressed the main risk of diabetes contributing to the infection. Chen et al 31 and Yoon et al 10 reported liver abscess as the most frequent source of endogenous endophthalmitis, with Klebsiella pneumoniae as the most common pathogen.…”
