Summary The combination of tamoxifen and transdermal hormone replacement therapy (HRT) may potentially reduce risks and side-effects of either agent, but an adverse interaction could attenuate their beneficial effects. We assessed the effects of their combination on cardiovascular risk factors within a prevention trial of tamoxifen. Baseline and 1 2-month measurements of total, low-density lipoprotein (LDL)-and high-density lipoprotein (HDL)-cholesterol, platelets and white blood cells were obtained in the following four groups: tamoxifen (n = 1117), placebo (n = 1112), tamoxifen and HRT (n = 68), placebo and HRT (n = 87). The analysis was further extended to women who were on HRT at randomization but discontinued it during the 12-month intervention period (n= 33 on tamoxifen and n = 35 on placebo) and to women who were not on HRT but started it during intervention (n = 36 in both arms of the study). Compared with small changes in the placebo group, tamoxifen was associated with changes in total, LDL-and HDL-cholesterol of approximately -90/, -19% and +0.2% in continuous HRT users compared with -9%/6, -14% and -0.8% in never HRT users. Similarly, there was no interaction on platelet count. In contrast, the decrease in total and LDL-cholesterol levels induced by tamoxifen was blunted by two-thirds in women who started HRT while on tamoxifen (P = 0.051 for the interaction term). We conclude that the beneficial effects of tamoxifen on cardiovascular risk factors are unchanged in current HRT users, whereas they may be attenuated in women who start transdermal HRT while on tamoxifen. Whereas a trial of tamoxifen in women already on transdermal HRT is warranted, prescription of HRT during tamoxifen may attenuate its activity.

A importância da terapia de reposição hormonal da menopausa (TRHM) na qualidade de vida na pós-menopausa é inquestionável. Entretanto, nem todos os efeitos determinados pelo seu emprego estão bem estabelecidos. Este estudo tem como objetivo avaliar a influência da TRHM sobre os níveis séricos do hormônio do crescimento (GH), fator de crescimento semelhante à insulina I (IGF-I), proteína ligadora 3 do IGF (IGFBP-3), glicose e insulina. Realizamos um ensaio clínico controlado, prospectivo, longitudinal e comparativo, no qual 53 mulheres na pós-menopausa, natural ou cirúrgica, foram submetidas ou não a TRHM contínua, durante um período de 6 meses, com estrogênios conjugados (EC-0,625mg/d) associados ou não ao acetato de medroxiprogesterona (AMP-2,5mg/d), pela via oral. As participantes foram subdivididas em 3 grupos: Grupo EC + AMP > 20 mulheres com útero, que utilizaram EC e AMP; Grupo EC > 20 mulheres histerectomizadas, que usaram EC; Grupo C > 13 mulheres, sem TRHM. Ao início e ao final do estudo foram realizadas as dosagens basais do GH; IGF-I e IGFBP-3. Também realizamos o teste oral de tolerância à glicose (TOTG) de 2 horas, com determinação dos níveis de glicose e insulina; a área abaixo da curva (AAC) de glicose e de insulina e o índice de resistência à insulina (IRI). O emprego da TRHM reduziu os níveis do IGF-I no EC + AMP (p= 0,01) e EC (p= 0,0007), sem alteração nos níveis do IGFBP-3. Os níveis do GH se elevaram mediante a TRHM, (EC + AMP: p= 0,004 e EC: p= 0,0003), entretanto, as concentrações séricas do IGF-I e do IGFBP-3 não parecem ser bons marcadores da secreção circadiana do GH. Aos 6 meses observou-se uma correlação negativa do IGF-I com a AAC de glicose nos três grupos (EC + AMP: r= -0,42, p= 0,06; EC: r= -0,58, p= 0,007 e C: r= -0,64, p= 0,01). O IGFBP-3 e a AAC de glicose apresentaram correlação negativa no grupo EC (r= -0,45, p= 0,04) e tendência no EC + AMP (r= -0,42, p= 0,06). A associação do AMP determinou o aparecimento de ITG em 30% das pacientes do grupo EC + AMP (n= 6). Nossos dados sugerem uma interação entre o metabolismo dos carboidratos com o IGF-I e o IGFBP-3. Os efeitos gerados pelo emprego prolongado da TRHM contínua na regulação do GH, IGF-I e IGFBP-3 ainda necessitam elucidação. The climacteric is not a condition of the modern age, although with increased life expectancy over the centuries, more women will experience this physiological transition. As women are living longer there is a greater expectation that good health will be maintained through to the late decade. Thus the potential long-term adverse health consequences of using hormonal therapies (HTs) to alleviate menopausal symptoms are of considerable concern for women and medical practitioners. This concern is often the basis for a decision whether or not to use HT.We have reviewed the history of knowledge of the menopause and the development of HT for the treatment of climacteric complaints. We have also summarised the current evidence for specific benefits and risks of HT. Data indicate that postmenopausal HT is appropriate for the management of vasomotor symptoms, but that HT should not be prescribed for the prevention of cardiovascular disease or dementia. HT does prevent bone loss and osteoporotic fracture; however, use for this purpose remains controversial. The risk of breast cancer with HT varies according to the preparation used, such that oestrogen without concurrent progestin appears to convey little, or possibly even no significant breast cancer risk. There is insufficient information regarding the long-term use of non-oral HT, low-dose HT or novel compounds such as tibolone or the selective oestrogen receptor modulators with respect to breast cancer and cardiovascular risk for specific recommendations to be made.
