68Ga-Labeled PSMA Uptake in Nonprostatic Malignancies

Abstract: PET/CT with Ga-labeled prostate-specific membrane antigen (PSMA) is increasingly recognized as the best imaging modality for disease staging and detection of recurrent prostate cancer. Despite its name, PSMA expression has been reported in a number of nonprostatic benign and malignant pathologies. Apparently, angioneogenesis is the mechanism attributed to increased Ga-PSMA uptake at these sites. Here we illustrate the utility of Ga-PSMA in 5 nonprostatic malignancies that could open up new possibilities for di… Show more

“…However, PSMA PET is not as specific for prostate cancer as previously thought. An increasing number of nonprostatic disorders have shown PSMA expression (79,80), including inflammatory/infectious diseases, vascular conditions, benign neoplasms, and other malignancies. Awareness of these findings may prevent misinterpretation.…”

“…7 First described in studies of monoclonal antibodies to a novel antigenic marker in PC in the late 1980s, [8][9][10] it has rapidly become clear that despite its name, the expression of this transmembrane protein is not restricted to PC. It also can be found in normal prostatic tissue and the vascular endothelium in a wide variety of solid tumors, 11,12 but not in blood vessels of normal tissues. Physiologic PSMA expression is reported in the lacrimal and salivary glands, the liver, the spleen, the kidneys, and the intestines.…”

“…68 Ga-PSMA, which is a tumor-specific antigen that was first used in prostate cancer staging, has been repeatedly utilized and is rapidly becoming a complementary tracer for breast cancers with low FDG avidities. High PSMA uptake has been exhibited in invasive ductal carcinoma, triple-negative breast cancer, and rare pathological subtypes such as Signet-ring cell breast adenocarcinoma ( 110 112 , 127 ). Like 18 F-FDG, tracers based on cellular metabolism are continuously being studied.…”
