Background: Bariatric procedures are effective options for weight loss (WL) in the morbidly obese. However, some patients fail to lose any weight after bariatric surgery, and mid-term weight maintenance is variable. The aim of this study was to investigate whether initial WL could predict mid-term weight maintenance. Methods: Eighty patients were enrolled, of whom 44 were treated with the BioEnterics Intragastric Balloon (BIB), 21 with laparoscopic adjustable gastric lap-banding (LAGB), and 15 with laparoscopic sleeve gastrectomy (LSG). Percentage of body WL and percentage of excess weight loss (EWL) were calculated at baseline and after 1, 3, 6, and 12 months. Successful WL was defined as EWL > 20% for patients treated with BIB and > 50% for patients treated with LAGB and SG. Results: Success in the 6th and 12th month was achieved in 80% and 58% of patients in the BIB group, 33% and 40% in the LAGB group, and 60% and 73% in the LSG group. In the BIB group, WL in the 1st month correlated positively with WL at the 6th and 12th month, and an initial WL > 6.5% best predicted success (sensitivity 50%, specificity 80%). A similar association was observed in the LAGB group at the 6th and 12th month and an initial WL > 9.4% best predicted success (sensitivity 90.0%, specificity 81.2%). In patients treated with LSG, WL in the 3rd month correlated positively with EWL at the 6th and 12th month, with a cutoff value of 17% (sensitivity 66.7%, specificity 100%). Conclusions: WL in the 1st month in patients treated with BIB and LAGB and WL in the 3rd month in patients treated with LSG could be used as a prognostic factor to predict mid-term weight maintenance.

Dear editor, Please consider these comments in response to the Letter to the Editor submission raising concerns about our recently published article by Yorke et al., BIntragastric balloon for management of severe obesity: a systematic review [1].Ŵ e appreciate the interest taken to our article and acknowledge the peer-reviewed critique. Our paper reflects the issue of bridging morbidly obese patients to definitive bariatric surgery with the placement of intragastric balloons (IGB). Our interest was to capture studies that reflected the use of IGB for bariatric specific weight loss only. This was reflected in our search criteria requiring both search terms for intragastric balloon and for bariatric surgery. While we understand that this narrowed focus limits the study's external validity and hence the generalizability of its findings, our question, as bariatric surgeons, was for bariatric specific weight loss.The main concern in the Letter to the Editor was the omission of the paper by Kotzampassi et al. in our systematic review. While it indeed might be the third largest paper existing on the efficacy and safety of IGB, it simply did not meet our specific search criteria [2]. It is, however, interesting and noteworthy that the findings of our study are concordant with the endpoints in that particular study.A few other issues were raised about methodological and citation techniques. As experienced systemic reviewers of electronic databases, we take great care in our methodologically sound principles and therefore we started our database search with the earliest year possible for PubMed citations, 1946. We can appreciate that content matter specialists might start the search upon the implementation of the IGB; however, in order to be systematic and unbiased in our search strategy, we implemented systematic and reproducible principles. The note about the inaccurate citation is valid.We are hopeful to the future of intragastric balloons for bariatric and non-bariatric weight loss, and look forward to reading any future research in this area from the Kotzampassi et al. team. Az elhízás és kísérő betegségeinek kezelése jelentős terhet ró az egészségügyi rendszerre. Hatékony konzervatív kezelés hiányában egyre nagyobb teret kapnak az invazív kezelési lehetőségek. Az elhízás kezelésében alkalmazott sebé-szi és endoszkópos beavatkozások alapvetően két módszeren alapulnak, amelyek a táplálékfogyasztás restrikciója, illetve malabszorpció révén vezetnek a testsúly csökkenéséhez. A szerző áttekinti a leghatékonyabb sebészi és endoszkópos módszereket az elhízás kezelésében. Orv. Due to the lack of efficacy of conservative treatment methods, attention has shifted towards invasive methods. Surgical and endoscopic treatments of obesity are based on two different methods: restrictive and malabsorptive procedures or their combination. The author reviews the most effective surgical and endoscopic procedures in the treatment of obesity. A XXI. század mérföldkő a testsúly szempontjából: az emberiség történetében először fordult elő, hogy a túl-súlyosak és az alultápláltak aránya a teljes populációban az előbbiek javára billent. Európában a 2013. évi OECD statisztikai adat szerint Magyarország a "legkövérebb" nemzet és világviszonylatban is dobogós helyen vagyunk. A hazai lakosság közel kétharmada túlsúlyos vagy elhí-zott. A társbetegségek miatt a 25 éves korban kórosan elhízott férfiak életkilátásai 22%-kal rosszabbak normális testsúlyú társaiknál, átlagosan 12 évvel élnek kevesebbet.A jelenlegi tendenciák folytatódásával 2030-ra Ameriká-ban további 65 millió, Nagy-Britanniában 11 millió túl-súlyos, elhízott, a két országban együtt 6-8,5 millió új diabeteses, 5,7-7,3 millió új szívbeteg, sőt 492-699 ezer új daganatos beteg várható [2,3]. Ennek természetesen gazdasági vonzatai sem elhanyagolhatóak. Konzervatív terápiás lehetőségekA testtömeg 5%-os csökkenése már kimutatható javulást eredményez a társbetegségek, elsősorban a vérzsír, a magas vérnyomás, cukorbetegség tekintetében. A konzervatív terápiák esetén ez a kalóriabevitel csökkentését (40 kg/m 2 testtömegindexű nő esetében ez átlagosan napi 350 kcal mindössze!) és a fizikai aktivitás növelését jelenti. A WHO szerint heti 150 perc testmozgás kedvező hatású, és ha a mozgás a napi 60 percet eléri, az eredmé-nyek még jobbak [4].Ha ilyen egyszerű a helyzet, akkor indokolt a kérdés: szükség van-e sebészeti vagy endoszkópos invazív beavatkozásra a testsúly csökkentésére?
