Non-clear cell renal cell carcinoma (nccRCC) is not as common as clear cell renal cell carcinoma (ccRcc). But then it is not uncommon in regular urological practice. It usually carries a grave prognosis. Authors need to converse ourselves with National comprehensive cancer network (NCCN) categories for evidence and we need to converse as well with NCCN categories for preferences. ‘Preferred intervention’ is based on superior efficacy, safety and evidence and when appropriate, affordability. ‘Other recommended interventions’ are somewhat less efficacious, more toxic, or based on less mature data or significantly less affordable for similar outcomes. ‘Useful in certain circumstances’ are interventions that may be used for a selected patient population. Clinical trials for targeted agents are mainly directed at ccRCC because of its high prevalence. According to the NCCN panel, enrolment in clinical trials is the preferred strategy for nccRCC. Outcomes of patients with nccRCC have improved with the introduction of targeted therapy. Precise pathological diagnosis of the types of nccRCC by immunohistochemical analysis is mandatory. This enables specific treatments for individual nccRCC. Currently TKIs are the drug of choice (both first and second line) for metastatic papillary RCC. Both TKIs and mTOR inhibitors are effective against chromophobe RCC. Platinum based chemotherapy should be used for metastatic CDC. Further evidence is required for management of nccRCC.

Purpose The outcome of RCC has improved considerably in the last few years, and the treatment options have increased. LACOG-GU and LARCG held a consensus meeting to develop guidelines to support the clinical decisions of physicians and other health professionals involved in the care of RCC patients. Methods Eighty questions addressing relevant advanced RCC treatments were previously formulated by a panel of experts. The voting panel comprised 26 specialists from the LACOG-GU/LARCG. Consensus was determined as 75% agreement. For questions with less than 75% agreement, a new discussion was held, and consensus was determined by the majority of votes after the second voting session. Results The recommendations were based on the highest level of scientific evidence or by the opinion of the RCC experts when no relevant research data were available. Conclusion This manuscript provides guidance for advanced RCC treatment according to the LACOG-GU/LARCG expert recommendations.

Purpose of review The treatment landscape of advanced renal cell carcinoma (RCC) continues to shift as both new targeted therapies and immunotherapies show efficacy in treating the disease. Contemporary insights into the molecular characterization of RCC are likely to fuel the development of additional therapies. This review summarizes recent advancements in the biologic characterization of RCC and discusses newly approved therapies and ongoing studies in the treatment of advanced RCC. Recent findings The Cancer Genome Atlas has now completed comprehensive molecular characterization of clear cell, papillary and chromophobe RCC, providing insights into the biology of these entities. Two new ‘targeted’ therapies, cabozantinib and lenvatinib, as well as a novel immune checkpoint inhibitor, the PD-1 inhibitor nivolumab, have recently been approved for the treatment of metastatic RCC. Although some of these newer therapies are associated with prolongation of survival, there are few long-term responders and the quest for more durable treatment strategies continues. Summary The addition of several new agents effective in metastatic renal cell carcinoma has resulted in improvements in overall survival; however, there are few avenues to durable responses or cure. Ongoing studies as well advances in our understanding of the molecular alterations underlying distinct forms of RCC promise further therapeutic advances and have the potential to alter the current treatment paradigm.
