410 Oral dysbiosis in palmoplantar pustulosis patients with arthro-osteosis

Abstract

Our retrospective study of 85 palmoplantar pustulosis (PPP) patients suggested the association with oral focal infection including dental infection and tonsillitis. To demonstrate oral dysbiosis in PPP, we analyzed the salivary microbiome of 12 patients (pts) and 10 healthy controls (HCs) by sequencing of 16S rRNA gene. Initially, we compared the bacterial community composition of pts and HCs by using UniFrac, a phylogenic tree-based metric ranging from 0 (identical communities) to 1 (completely different communities). The UniFrac distance between pts-HCs (0.42) was significantly higher than HCs-HCs (0.38) and pts-pts (0.37) (P<0.05), indicating oral dysbiosis in pts. A principle coordinate analysis (PCoA) plot also showed clearly different distribution of pts. Next, we analyzed the UniFrac distance among the pts with different characteristics. Whereas PPPASI score was not associated with oral microbiota, pts with smoking habits, periodontitis and pustulotic arthro-osteosis (PAO) showed significant difference. The analysis of bacterial abundance in pts and HCs demonstrated lower Proteobacteria (pts¼24%, HCs¼37%, P<0.05) at the phyla level, and lower Haemophilus (pts¼10%, HCs¼17%, P<0.05) and higher Prevotella (pts¼17%, HCs¼10%, P<0.05) at the genus level in pts. Interestingly, these differences were observed in pts with PAO (n¼7; Proteobacteria: pts+PAO¼21%, HCs¼37%; Haemophilus: pts+PAO¼8%, HCs¼17%; Prevotella: pts+PAO¼20%, HCs¼10%; P<0.05), while not in pts without PAO (n¼5). Taken together, these results suggested oral dysbiosis in PPP, particularly in the patients with PAO.
