Potential role of 3DUS in infants and children

The objective of this review is to discuss basic technical aspects as well as potential pediatric applications of three-dimensional ultrasound (3DUS). Different 3DUS-techniques are already commercially available. Most commonly, a transducer-integrated motor drives a scan-head to acquire a volumetric dataset, which is then reconstructed and viewed using various post-processing techniques. It has been proved feasible to apply 3DUS with pediatric transducers in typical pediatric investigations. Based on our own experiences and on review of the literature, 3DUS can be successfully used in infants and children for brain, spine, cardiac, urinary tract (particularly assessment of hydronephrosis, and virtual cystoscopy), and female genital (e.g., for assessment of uterine malformations) US, and for various other applications, particularly in small parts. Power Doppler data can be integrated, allowing 3DUS-angiography. Thus, 3DUS promises to become a useful adjunct for imaging children, particularly as it enhances ultrasound by offering additional, previously inaccessible planes, rendering options and surface assessments. 3DUS should be increasingly exploited, thus hopefully helping reduce the need for more invasive or burdening (e.g., ionizing radiation) investigations in children.

Zusammenfassung Gegenstand und Ziel Gewebeveränderungen lassen sich bei Überlagerungen durch Gas im Magen-Darm-Trakt oder die Rippenbegrenzung sonografisch nicht darstellen, sind dagegen in der CT gut darstellbar. Die Verknüpfung von Sonografie und CT kann eine verbesserte Aussagekraft im Vergleich zur alleinigen Sonografie schaffen und als Volumennavigation im registrierten Datensatz als Hilfsmittel eingesetzt werden. Zur Diagnosesicherung eines verdächtigen Herdes ist eine Gewebeprobe nötig, die unter sonografischer Kontrolle entnommen werden kann. Auch hier lässt sich die ortskorrekte Fusion CT/Sonografie nutzen, um durch Ausschaltung von Überlagerungen im sonografischen Bild – eine Limitation der Sonografie – Herde besser darzustellen. Ob die Fusion CT/Sonografie exakt ist und welches Registrierungsprotokoll die höchste Genauigkeit aufweist, wurde in der vorliegenden Studie untersucht. Material und Methoden Von 16 hergestellten Modellen (10 Modelle mit Erbsen [Low-Contrast-Modell], 6 Modelle mit Holzkugeln [High-Contrast-Modell] als Rundstrukturen) wurden CT-Datensätze erstellt, anhand von 3 Protokollen der Volumennavigation fusioniert und sonografisch untersucht. Die Abweichung der Probekörper wurde gemessen. Ergebnisse Es erfolgten 1026 Messungen der Modelle mit Erbsen und 648 Messungen der Modelle mit Holzkugeln. Bei 9,9 % (102/1026) bzw. 9,9 % (64/648) der Modelle konnte eine 100 %ige Registrierungsgenauigkeit bestimmt werden. Bei 85,4 % (876/1026) bzw. 94,1 % (610/648) lag die Abweichung der Strukturen < 5 mm und bei 98,1 % (1006/1026) bzw. 99,4 % (644/648) < 10 mm. Das Registrierungsprotokoll, bei dem alle gesetzten Referenzpunkte für die Erstellung der ortskorrekten Fusion zwischen Ultraschall- und CT-Datensatz genutzt wurden, zeigte die genauesten Fusionsergebnisse. Schlussfolgerung Die Registrierungsprotokolle der volumennavigierten Sonografie weisen eine für Biopsien ausreichend hohe Genauigkeit bei der Fusion identischer Lokalisationen auf und stellen die Grundlage für eine verbesserte volumennavigierte Biopsie dar.

Computers used as supporting tools for diagnostics, operation planning and therapy are of increasing relevance in surgery. Rapid progress in imaging techniques such as computed tomography (CT), magnetic resonance imaging (MRT) and ultrasound already allows to represent anatomical and physiological conditions with maximal authenticity. In order to simulate complex surgeries we must develop ergonomic and intuitively useable software tools, thus enabling a precise and fast virtual execution of the planned surgical intervention preoperatively. Intraoperative support will consist of passive navigation tools, available already today, supporting the intraoperative orientation and, in the future, robots performing specific steps autonomously. Methods of augmented reality for the interaction of virtual objects and the real surgical scene are also suitable for the visualization of planning data and other medically relevant information in the operation situs. In maxillofacial and craniofacial surgery the techniques mentioned have been applied in all fields from dental implantology up to the correction of craniofacial malformations and the resection of skull base tumors. Many applications are still being developed or are still in the form of a prototype. However, it is already clear that developments in this area will have a considerable effect on a surgeon's routine work.
