This 12-week study investigates the effects of resistance training with different set structures on the plasma levels of brain-derived neurotrophic factor (BDNF), nerve growth factor (NGF), and obesity-related markers in middle-aged Korean women with obesity. A total of 40 middle-aged women with obesity (mean age, 59.87 ± 2.80 years) were enrolled in this study; only 31 women were able to complete the 12-week exercise period. The participants were randomly divided into the following four groups: control (CON, n = 8), drop set system (Drop, n = 8), descending set system (Descend, n = 7), and ascending set system (Ascend, n = 8). Body composition was recorded, and blood samples were obtained and evaluated before and after the 12-week exercise period intervention. Two groups showed no significant differences in body weight, body fat percentage, and body mass index before and after the 12-week exercise period. However, significant differences were observed in the blood levels of BDNF, NGF, adiponectin, leptin, and glucagon-like peptide-1 (GLP-1). BDNF and NGF showed significant differences in both time and interaction effects. Post hoc analysis revealed that the Drop group had higher BDNF and NFG levels than those of the Ascend and CON groups, while there was a significant increase in the levels of these biomarkers in the Descend and Drop groups in the time comparison. Adiponectin levels also showed significant differences in both time and interaction effects. Post hoc analysis revealed that the Drop and Descend groups had higher adiponectin levels than those of the CON group. Leptin levels decreased significantly in the Descend and Drop groups over time. GLP-1 levels showed no significant differences between the groups; however, there were significant differences in both time and interaction effects. Post hoc analysis revealed that the Drop group had lower GLP-1 levels than those of the CON group. This study suggests that resistance training with different set structures can have varying effects on the blood levels of different biomarkers in middle-aged women with obesity. These findings have implications for exercise prescription, and may provide insights into the mechanisms underlying the beneficial outcomes of resistance training in middle-aged Korean women with obesity.

SummaryObjectivesThis study assessed the transparency and replicability of exercise‐based interventions following bariatric surgery by evaluating the content reporting of exercise‐based clinical trials.DesignThe study design of the present article is a systematic review.Data sourcesPubMed, Scopus, Web of Sciences, PsycINFO, and Cochrane were searched from their inception to May 2023.Eligibility criteriaEligible studies were clinical trials including exercise interventions in participants following bariatric surgery. There were 28 unique exercise interventions. Two independent reviewers applied the exercise prescription components of Frequency, Intensity, Time, and Type (FITT; four items) and the Consensus on Exercise Reporting Template (CERT; 19 items). Exercise interventions were organized into four major exercise components: aerobic training, resistance training, concurrent training, and “others.”ResultsThe FITT assessment revealed that 53% of the trials did not report the training intensity, whereas 25% did not indicate the duration of the major exercise component within the training session. The mean CERT score was 5 out of a possible score of 19. No studies reached CERT score >10, while 13 out of the total 19 CERT items were not adequately reported by ≥75% of the studies.ConclusionThis study highlights that the exercise interventions following bariatric surgery are poorly reported, non‐transparent, and generally not replicable. This precludes understanding the dose–response association of exercise and health‐related effects and requires action to improve this scientific field.

A obesidade é definida como o acúmulo anormal ou excessivo de gordura que pode prejudicar a saúde. A medida mais utilizada para classificar a obesidade é o índice de massa corporal (IMC), calculado como o peso corporal em quilogramas dividido pela altura em metros ao quadrado (kg/m²). Em adultos, um IMC desejável está entre 18,5 a 25 e o sobrepeso está entre 25 a 30. A obesidade é definida como IMC acima de 30, enquanto a obesidade grave ou mórbida é definida como IMC acima de 40 (Colquitt et al., 2014).A cirurgia bariátrica (CB) é considerada uma intervenção eficaz para pacientes obesos, visando reduzir o peso e manter qualquer perda por meio da restrição da ingestão ou má absorção dos alimentos, ou a combinação das duas (Delgado-André et al., 2018). Para tal, atualmente, existem vários procedimentos cirúrgicos diferentes que têm sido usados (Colquitt et al., 2014).Essa cirurgia demonstrou resultar em grande perda de peso e manutenção do peso a longo prazo, mas muitos pacientes acham difícil se adaptar aos novos comportamentos de estilo de vida exigidos após a cirurgia (Sarwer, Wadden & Fabricatore, 2005), tornando-se comum o ganho de peso em um ano, após a cirurgia para esses pacientes, seguido por uma diminuição na qualidade de vida relacionada à saúde (Sellberg et al., 2018).Após a CB pode-se analisar, na literatura, que a atividade física (AF) pode melhorar a perda de peso, e os benefícios refletem nas mudanças no estilo de vida, durante os primeiros anos de acompanhamento (Gill et al., 2018). Nesse sentido, atender às recomendações de AF de pelo menos 150 minutos de moderada a vigorosa por semana está associado a uma ampla gama de resultados positivos para a saúde em todas as faixas de peso (Sellberg et al., 2019).Segundo os estudos de Nuijten et al. ( 2021) e Tettero et al. ( 2021), os pacientes bariátricos conseguiram melhorar a aptidão cardiorrespiratória, composição corporal e qualidade de vida, após a cirurgia, em comparação com pacientes que reduziram os níveis de AF.Percebe-se, através das evidências, que crescimento em técnicas invasivas não elimina totalmente os hábitos não saudáveis, onde as modificações no estilo de vida, tal como a alimentação saudável e programas de AF corretos, podem melhorar, de maneira concisa, os resultados cirúrgicos (Tabesh et al., 2019). Nesse contexto, o presente estudo buscou relatar os benefícios do exercício físico após cirurgia bariátrica.
