Objective. To offer a baseline for clinical diagnosis, echocardiography was performed to evaluate the disparities in heart function comparing pregnant women with diabetes mellitus (GDM) and ordinary pregnant women. Methods. A prospective case-control study is being conducted on pregnant women with or without gestational diabetes. The sample size for both the intervention and control groups is the same: no diabetes diagnosis or previous forms, a single pregnancy, and no issues (such as preeclampsia or fetal growth restriction). The females were all subjected to routine echocardiograms to examine the morphology and function of their left and right hearts. Results. In the research, 51 women with GDM and 50 healthy controls volunteered. Women with GDM had a significantly higher heartrate ( 82 ± 9 vs. 74 ± 8 ), left ventricular (LV) relative wall thickness ( 0.39 ± 0.06 vs. 0.31 ± 0.07 ; P < 0.001 ), LV early diastolic transmitral valve velocity (E) ( 0.79 ± 0.14 vs. 0.72 ± 0.13 m/s; P = 0.031 ), and LV late diastolic implementing regulations valve velocity (0.6). Speckle-tracking analysis showed significant decrease in LV right ventricular (RV). A study indicated a reduced pulmonary acceleration time ( 59 ± 9 vs. 68 ± 12 ms; P = 0.001 ), RV E/A ratio ( 1.21 ± 0.19 vs. 1.31 ± 0.31 ; P = 0.022 ), and a greater RV myocardial systolic annular velocity ( 0.17 ± 0.03 vs. 0.12 ± 0.03 ; P = 0.023 ). Conclusions. Our results revealed that the heart function of diabetic pregnant women differed considerably from that of the control group, such as LV-RWT, LV diastolic transmitral valve speed, and LV late diastolic transmitral valve speed. Given these results, further research into the postpartum cardiovascular healing of pregnant women with gestational diabetes mellitus is required.

ObjectivesTo assess the diagnostic accuracy of non-invasive or minimally invasive autopsy techniques in deaths under 1 year of age.DesignThis is a systematic review of diagnostic test accuracy. The protocol is registered on PROSPERO.ParticipantsDeaths from conception to one adjusted year of age.Search methodsMEDLINE (Ovid), EMBASE (Ovid), CINAHL (EBSCO), the Cochrane Library, Scopus and grey literature sources were searched from inception to November 2021.Diagnostic testsNon-invasive or minimally invasive diagnostic tests as an alternative to traditional autopsy.Data collection and analysisStudies were included if participants were under one adjusted year of age, with index tests conducted prior to the reference standard.Data were extracted from eligible studies using piloted forms. Risk of bias was assessed using Quality Assessment of Diagnostic Accuracy Studies-2. A narrative synthesis was conducted following the Synthesis without Meta-Analysis guidelines. Vote counting was used to assess the direction of effect.Main outcome measuresDirection of effect was expressed as percentage of patients per study.FindingsWe included 54 direct evidence studies (68 articles/trials), encompassing 3268 cases and eight index tests. The direction of effect was positive for postmortem ultrasound and antenatal echography, although with varying levels of success. Conversely, the direction of effect was against virtual autopsy. For the remaining tests, the direction of effect was inconclusive.A further 134 indirect evidence studies (135 articles/trials) were included, encompassing 6242 perinatal cases. The addition of these results had minimal impact on the direct findings yet did reveal other techniques, which may be favourable alternatives to autopsy.Seven trial registrations were included but yielded no results.ConclusionsCurrent evidence is insufficient to make firm conclusions about the generalised use of non-invasive or minimally invasive autopsy techniques in relation to all perinatal population groups.PROSPERO registration numberCRD42021223254.

ObjectiveTo explore the application of the proposed intelligent image processing method in the diagnosis of aortic coarctation computed tomography angiography (CTA) and to clarify its value in the diagnosis of aortic coarctation based on the diagnosis results.MethodsFifty-three children with coarctation of the aorta (CoA) and forty children without CoA were selected to constitute the study population. CTA was performed on all subjects. The minimum diameters of the ascending aorta, proximal arch, distal arch, isthmus, and descending aorta were measured using manual and intelligent methods, respectively. The Wilcoxon signed-rank test was used to analyze the differences between the two measurements. The surgical diagnosis results were used as the gold standard, and the diagnostic results obtained by the two measurement methods were compared with the gold standard to quantitatively evaluate the diagnostic results of CoA by the two measurement methods. The Kappa test was used to analyze the consistency of intelligence diagnosis results with the gold standard.ResultsWhether people have CoA or not, there was a significant difference (p < 0.05) in the measurements of the minimum diameter at most sites using the two methods. However, close final diagnoses were made using the intelligent method and the manual. Meanwhile, the intelligent measurement method obtained higher accuracy, specificity, and AUC (area under the curve) compared to manual measurement in diagnosing CoA based on Karl's classification (accuracy = 0.95, specificity = 0.9, and AUC = 0.94). Furthermore, the diagnostic results of the intelligence method applied to the three criteria agreed well with the gold standard (all kappa ≥ 0.8). The results of the comparative analysis showed that Karl's classification had the best diagnostic effect on CoA.ConclusionThe proposed intelligent method based on image processing can be successfully applied to assist in the diagnosis of CoA.
