Histamine mediates numerous physiological processes. It is crucial in controlling allergic reactions, immune system responses, and also plays a significant role in allergic inflammation. Histamine mainly exhibits its effects in the human body by mediating through receptors such as H1, H2, H3, and H4. H1 receptor is widely distributed in major cells types such as immune and inflammatory cells. The binding of histamine to this receptor the chronotropic and vasoconstriction effects, rendering H1 receptor as an essential therapeutic target for medications that treat various allergic illnesses, including allergic rhinoconjunctivitis, urticarial, or atopic dermatitis 1 . H1 AHs, structurally distinct from histamine, stabilize the receptor's inactive conformation by acting as an inverse agonist. The first and second-generation H1 antihistamines (AHs) are inverse agonists that stabilize the inactive conformation of the receptor in its active state. The Union of Pediatricians of Russia together with the Russian Association of Allergologists and Clinical Immunologists and the Russian Society of Dermatovenerologists and Cosmetologists have developed new clinical guidelines for the urticaria in adults and children. Urticaria is a common disease; its various clinical variants are diagnosed in 15–25% of people in the global population, and a quarter of all cases belongs to chronic urticaria. The prevalence of acute urticaria is 20%, and 2.1–6.7% in child population, whereas acute urticaria is more common in children than in adults. The prevalence of chronic urticaria in adults in the general population is 0.7 and 1.4%, and 1.1% in children under 15 years of age, according to the systematic review and meta-analysis, respectively. This article covers features of epidemiology, etiology, and pathogenesis of the disease with particular focus on differential diagnostic search. Guidelines on treatment and step-by-step therapy scheme (both based on principles of evidencebased medicine) for pediatric patients were presented. Clarification on the analysis of the therapy efficacy and the degree of disease activity was given.

Antihistamine over-the-counter (OTC) drugs for allergic rhinitis are widely used and cause central nervous system side effects. Most available data on anti-allergic drugs are on controlled usage. It is necessary to investigate the occurrence of side effects in the context of self-medication to avoid inappropriate use. We aimed to clarify the association between the usage of OTC anti-allergic drugs and central nervous system side effects. Patients and Methods: An online, anonymous, cross-sectional study was conducted using a structured questionnaire. People who had used OTC anti-allergic drugs in the year prior to the study were recruited. To assess the association between inappropriate drug use and the occurrence of side effects, a binary logistics regression analysis was performed according to three dosage forms (oral only, nasal only, and oral and nasal combined use). Results: Somnolence was experienced by 59.1% of the participants using the OTC drug for allergic rhinitis.
