International, multicenter retrospective trial comparing the efficacy and safety of bi-flanged versus lumen-apposing metal stents for endoscopic drainage of walled-off pancreatic necrosis

Abstract: Background To compare fully covered bi-flanged metal stents (BFMS) and lumen-apposing metal stents (LAMS) for endoscopic ultrasound (EUS)-guided drainage/debridement of pancreatic walled-off necrosis (WON). Methods Patients with WON managed by EUS-guided therapy were divided into those who underwent: 1) drainage using BFMS; and 2) drainage using LAMS and scheduled direct endoscopic necrosectomy (DEN). Clinical success (resolution of the WON), technical success (successf… Show more

“…44 A recent RCT of 387 patients undergoing WON drainage reported a higher rate of occlusion with SEMSs (10.2%) compared with LAMSs (5.9%). 61 Other studies have reported stent occlusion rates ranging between 0% and 10.2%, 46,47,62 with higher rates of occlusion and secondary infection reported with PSs over LAMSs. 47 More details on stent occlusion are discussed in the ASGE guideline on the role of endoscopy in the diagnosis and treatment of inflammatory PFCs.…”

“…In contrast, we had our meta-analysis’s 15 most recent full-text articles and independently compared LAMS and BFMS outcomes in a subgroup analysis. Studies comparing BFMS and LAMS for WON treatment have shown that the two methods produce similar results with fewer AE [ 36 , 37 ]. Our subgroup analysis also showed the same outcome results for the LAMS and the DPS, but only three studies reporting the outcome of the BFMS compared with the DPS were included in our analysis.…”

“…However, stent migration occurred significantly more often using the traditional type of FC-SEMSs, with all clinically significant dislodgements of traditional FC-SEMSs occurring during direct endoscopic necrosectomy. [ 71 ]…”
