Predictors and Clinical Impact of Late Ventricular Arrhythmias in Patients With Continuous-Flow Left Ventricular Assist Devices

Abstract: Late VAs are common after LVAD implantation. The VT-LVAD score may help to identify patients at risk of late VAs and guide ICD indications in previously nonimplanted patients. (Determination of Risk Factors of Ventricular Arrhythmias [VAs] after implantation of continuous flow left ventricular assist device with continuous flow left ventricular assist device [CF-LVAD] [ASSIST-ICD]; NCT02873169).

“…Arrhythmias in the immediate post-operative period are felt to be mediated by peri-operative adrenergic stimulation and the use of adrenergic agonists and positive cardiac remodeling on LVAD support leads to less arrhythmias later. Patients with a history of ventricular arrhythmias pre-LVAD are at increased risk even post-implant [66]. There is some data on the lack of beta blocker and ACE inhibitor use and etiology of cardiomyopathy as risk factors.…”

“…The ASSIST‐ICD (Clinicaltrials.gov identifier: NCT02873169) is a retrospective, multicenter observational study of durable mechanical circulatory support devices implanted in 659 patients treated in 19 tertiary care centers in France. The study methods have been previously reported 6 . Notably, it was initially conceived with the scope to address issues connected with decision‐making in the arrhythmology and cardiac electrophysiology field among LVAD recipients (rate of the occurrence of ventricular arrhythmias under LVAD therapy and impact of such arrhythmia in LVAD recipients with/without implantable cardioverter defibrillator).…”

“…The ASSIST-ICD is a retrospective, multicenter observational study (NCT02873169) of durable mechanical circulatory support devices implanted in 19 French tertiary centers. The methods of this study have been previously published (11). Briefly, patients aged ≥18 years implanted with axial HeartMate 2 (Abbott, Chicago, Illinois), Jarvik2000 (Jarvik Heart, Inc., New York, New York), or centrifugal HeartWare pumps (Medtronic, Columbia Heights, Minnesota) between February 2006 and December 2016 were included in the final analysis.…”
