Randomized controlled trial of a brief cognitive‐behavioral strategies intervention for the pain, fatigue, and sleep disturbance symptom cluster in advanced cancer

Abstract: The brief CBS intervention had limited effects in this trial. However, findings regarding potential mediators affirm hypothesized mechanisms and provide insight into ways to strengthen future interventions to reduce the suffering associated with co-occurring pain, fatigue, and sleep disturbance.

“…They resulted in the reduction of anxiety and in positively affecting quality of life, cancer treatment side effects and health behaviour (Greer et al, 2012; Guo et al, 2013; Willems et al, 2017). One intervention type is relaxation therapy, which shows good effects on reducing emotional stress (Arakawa, 1997; Kwekkeboom et al, 2018) and physical tension, which intensifies symptom perception (Kwekkeboom et al, 2018).…”

“…Guided relaxation appears to be effective in the treatment of pain [ 34 ••, 35 , 36 ] while the findings regarding self-administered relaxation show inconsistencies [ 37 , 38 •]. One study examined the efficacy of progressive muscle relaxation (PMR) and interactive guided imagery (IGI) on pain in patients with advanced cancer [ 34 ••].…”

“…While the aforementioned studies explored the effects of live guided relaxation, Kwekkeboom et al examined the efficacy of self-administered mp3-recorded exercises consisting of relaxation, imagery, and distraction techniques. In a pilot study [ 37 ] and a full-scale RCT [ 38 •], advanced cancer patients received an introductory therapeutic session and were encouraged to complete at least 1 of the 12 exercises on their own each day. Two weeks later, pain severity decreased in the intervention group of the pilot study in comparison with the waitlist controls.…”

“…Identifying the interrelationship between symptoms in a cluster can aid in more effective symptom management. For instance, symptoms may cluster together through a shared etiology such as neuroinflammation ( 55 ) or because they share the same triggering symptom ( 56 ). Identifying symptom clusters in individuals who report persistent symptoms following treatment for LB could aid in more effective symptom management through identifying triggering symptoms or an underlying etiology.…”
