Psychometric properties of discourse measures in aphasia: acceptability, reliability, and validity

Abstract: The current study is the first to psychometrically profile measures of discourse in aphasia. It contributes to the field by identifying Story Grammar, Topic Coherence, Reference Chains and PAS as the most psychometrically robust discourse measures yet profiled with speakers with aphasia. Until further data are available indicating the strength of other discourse measures, caution should be applied when using them.

“…Further research by Pritchard et al . (2018) identified story grammar, topic coherence, reference chains and predicate argument structure as being psychometrically robust. Time is a pervasive influential factor that warrants careful consideration in developing and advocating any such protocol.…”

“…are becoming more frequent and urgent. 11,14 We have demonstrated the utility of MCA, CoreLex, and efficiency measures to differentiate individuals with stable and progressive aphasia from healthy controls, as well as preliminary results of these measures for treatment outcomes. We have provided additional tools that can be used alongside MCA checklists and norms, 6,57,64 CoreLex checklists and norms, 65,68 and the AphasiaBank protocol 47,48 to sensitively and reliably assess discourse.…”

“…2,3 Subsequently, investigations have sought to develop informative measures [4][5][6][7][8] and establish and/or characterize psychometric strengths and weaknesses. [9][10][11] Importantly, treatment studies are more frequently investigating the impact of interventions on discourse outcomes in stable, strokeinduced aphasia. [12][13][14] Comparatively less attention has been paid to treatment outcomes, including discourse, in primary progressive aphasia (PPA).…”

“…For the individual picture stimuli from the AphasiaBank stimuli, she found poor test-retest reliability for measures of word retrieval errors; however, combined analysis of the different narrative tasks yielded some relatively stable measures (e.g., semantic and phonological errors). Relatedly,Pritchard et al (2018) provided select psychometric information,…”
