Aims: To systematically describe the factors influencing the perception of feeling safe among patients receiving pre-hospital emergency care. Background: Patient safety is a top priority worldwide. Little is known about how patients perceive safety during emergency health care and what the related factors are. Design: A mixed-methods systematic review.Methods: Six databases were searched to December 2021. Inclusion criteria were as follows: studies with patients of any age, framed in pre-hospital emergency care settings, referring to patient safety, addressing the perception of safety, and with a quantitative or qualitative research design. Exclusion criteria were as follows: conference records, abstracts, supplements, letters, editorials and reviews. The quality of the evidence was assessed with the Joanna Briggs Institute (JBI) checklists. A convergent integrated approach was followed according to the JBI methodology. The protocol was registered on PROSPERO (CRD42022306337). PRISMA 2020 guidelines were followed in reporting this review.Results: Among the 5223 records identified, the whole texts of 35 articles were reviewed. Ten studies were relevant to the research question. Seven followed a qualitative methodology, with interviews or focus group. Three involved a quantitative methodology, with questionnaires or validated measures. The 74 findings were organised into 11 categories and then grouped into four dimensions: 'receiving a satisfying response from healthcare professionals when I need it', 'EMS personnel's professional competence', 'the variable and unfamiliar settings of pre-hospital emergencies', and 'patients' personal characteristics'. Conclusions:The perception of safety is influenced by many factors, which can be grouped into four independent dimensions. Quantitative and qualitative studies can provide us with complementary findings. INTRODUCCIÓN: En Colombia, el tema de la humanización de los servicios de salud se encuentra estrechamente ligado a los aspectos normativos, los cuales datan de la conceptualización de la Constitución Política del país y otras normas anteriores que reglamentan la conducta médica y del personal que presta servicios de salud. OBJETIVO: Describir los aspectos relacionados con la humanización en los servicios de urgencia, en publicaciones científicas, mediante una revisión narrativa. MÉTODO: Se realizó una búsqueda bibliográfica, de artículos originales de acceso abierto publicados entre 2010 y 2020 en inglés, español y portugués. La búsqueda se concentró en los meses de marzo a julio de 2020 en cinco bases de datos disponibles en el servicio de biblioteca de la Universidad de Cartagena. RESULTADOS: Se recuperaron 17 artículos que aportaron significativamente al análisis y comprensión de la situación; el 100% en idioma inglés, de estudios desarrollados en cuatro continentes (América, Europa, Asia y Oceanía); los profesionales de enfermería cuentan con mayor número de publicaciones orientadas al conocimiento de la humanización de los servicios de salud. CONCLUSIÓN: Los resultados demuestran un necesario cambio de actitudes frente a la humanización, capacitación y reconocimiento del otro como ser humano. Es frecuente que se normalice la violencia en los servicios de urgencia, situación que es contraria a los criterios de humanización de los servicios de salud.

The Emergency Medical Services (EMS) are emergency services generally been designed to provide urgent treatment of patients with life-threatening conditions outside medical facilities. Even though the EMS belongs to the category of socio-economic rights, it nevertheless has great significance in safeguarding one of the most fundamental human rights, the right to life. In fact, international humanitarian law has recognised this important connection by establishing explicit legal rules that oblige states to ensure urgent medical care for the wounded and sick. International human rights law, on the other hand, has no such expressed provisions. However, the problem is not the lack of legal rules applicable to the EMS as such but rather the challenges in human rights perception, which hinder the EMS being perceived as a valuable human right. Therefore, this article essentially argues that international human rights law does not recognise the EMS as a human right sufficiently and that more thorough actions are required from the UN Committee of Economic, Social and Cultural Rights (CESCR) in this regard.
