2D shear wave liver elastography by Aixplorer to detect portal hypertension in cirrhosis: An individual patient data meta‐analysis

Abstract: Background & Aims: Liver stiffness measured with 2-dimensional shear wave elastography by Supersonic Imagine (2DSWE-SSI) is well-established for fibrosis diagnostics, but non-conclusive for portal hypertension. Methods:We performed an individual patient data meta-analysis of 2DSWE-SSI to identify clinically significant portal hypertension (CSPH), severe portal hypertension and large varices in cirrhosis patients, using hepatic venous pressure gradient

“…Other technologies for LSM, pSWE and 2D-SWE, may perform similarly to TE, but only 1 centre has performed a head-to-head comparison between TE and 2D-SWE (Supersonic Aixplorer) 56 and only 2 recent studies have assessed pSWE using the Virtual Touch technique (Siemens Acuson 2000), with no comparator. 57,58 It has been debated whether active use of alcohol may cause false positive LSMs.…”

“…The technical advances have nowadays elastography in any ultrasound machine and is routine in the exploration of the liver and also other organs. The best validated for portal hypertension is Fibroscan, which is recommended by Baveno to guide screening endoscopy for varices, followed by 2D-Shear-wave elastography of Aixplorer, the last being superior in patients with ascites (42,(138)(139)(140). The combination of liver and spleen stiffness measurement seems to be the most accurate to assess CSPH and the effect of portal pressure lowering strategies (42,125,139).…”

“…As for the advanced technology, share wave elastography (possible evaluation of liver/spleen stiffness with realtime observation even in patients with ascites), which is available for noninvasive prediction of the severity of portal hypertension and EV (87)(88)(89). However, it is an indirect assessment and lower specificity may be the disadvantage.…”
