Outcome of Neonates with Vein of Galen Malformation Presenting with Severe Heart Failure: A Case Series

Abstract: Aggressive intensive care approach to heart failure and pulmonary hypertension leading to early neurointervention results in good survival rates with low morbidity even in cases of high-risk neonatal VGAM. Combined hemodynamic treatment can improve outcome in neonates with cardiac failure secondary to VGAM, although there is the risk of precipitating systemic hypoperfusion and renal failure. A moderate prematurity may not prevent both interventional approach and good outcome.

“…A similar negative outcome was reported by another fetal report [ 25 ]. Another paper [ 26 ] reported on 12 neonates with aneurysm of the Galen vein, all with high output failure, treated by endovascular procedure, with success only in five patients, while the remaining seven patients died of refractory cardiac failure, multiorgan failure, and severe brain damage.…”

“…5 Therefore, embolization is the treatment of choice. 6 It should be attempted as soon as possible for neonates with VGAM presenting CHF and pulmonary hypertension, 7 and a BNE score between 8 and 12 out of 21 requires urgent therapy (►Table 1). 1 The transfemoral approach is commonly used for transarterial embolization; however, catheterization of the femoral artery can be difficult in neonates due to small vessels diameter.…”

“…El tratamiento ha aumentado la supervivencia, aunque aún se está intentando estimar el pronóstico neurológico de estos pacientes. 13 El pronóstico en la insuficiencia cardíaca secundaria a MAV cerebral durante el período neonatal es malo. En la serie de García-Mónaco et al, 14 revisaron el manejo de 30 niños con insuficiencia cardíaca por MAV cerebral y obtuvieron una mortalidad del 20 %.…”
