Comparison of Antiretroviral Therapy Adherence Among HIV-Infected Older Adults with Younger Adults in Africa: Systematic Review and Meta-analysis

Abstract: As access to antiretroviral treatment in low- and middle-income countries improves, the number of older adults (aged ≥ 50 years) living with HIV is increasing. This study compares the adherence to antiretroviral treatment among older adults to that of younger adults living in Africa. We searched PubMed, Medline, Cochrane CENTRAL, CINAHL, Google Scholar and EMBASE for keywords (HIV, ART, compliance, adherence, age, Africa) on publications from 1st Jan 2000 to 1st March 2016. Eligible studies were pooled for met… Show more

“…Studies in southern Africa have begun to apply life-course approaches to understanding HIV among older adults (9,16,(29)(30)(31)(32)(33)(34)(35). Findings have highlighted how age norms can make accessing HIV prevention, testing and care challenging, due to concerns about loss of respect and age-related stigma and discrimination reflecting experiences from earlier times when treatment was not widely available.…”

“…Among women in Uganda, older age and lower baseline CD4 cell count were associated with improved adherence. Older age is a factor frequently associated with greater ART adherence perhaps related to health literacy and life stability [57,58], although this observation largely focuses on those ages >50 years; the age range of this study was limited to those age ≤50. Studies of women taking HIV pre-exposure prophylaxis (PrEP) observe improved adherence with increasing age in younger cohorts [59], perhaps related to neurodevelopmental factors [60].…”
