IntroductionHyperplasia of the mesangial area is common in IgA nephropathy (IgAN) and diabetic nephropathy (DN), and it is often difficult to distinguish them by light microscopy alone, especially in the absence of clinical data. At present, artificial intelligence (AI) is widely used in pathological diagnosis, but mainly in tumor pathology. The application of AI in renal pathological is still in its infancy.MethodsPatients diagnosed as IgAN or DN by renal biopsy in First Affiliated Hospital of Zhejiang Chinese Medicine University from September 1, 2020 to April 30, 2022 were selected as the training set, and patients who diagnosed from May 1, 2022 to June 30, 2022 were selected as the test set. We focused on the glomerulus and captured the field of the glomerulus in Masson staining WSI at 200x magnification, all in 1,000 × 1,000 pixels JPEG format. We augmented the data from training set through minor affine transformation, and then randomly split the training set into training and adjustment data according to 8:2. The training data and the Yolov5 6.1 algorithm were used to train the AI model with constant adjustment of parameters according to the adjusted data. Finally, we obtained the optimal model, tested this model with test set and compared it with renal pathologists.ResultsAI can accurately detect the glomeruli. The overall accuracy of AI glomerulus detection was 98.67% and the omission rate was only 1.30%. No Intact glomerulus was missed. The overall accuracy of AI reached 73.24%, among which the accuracy of IgAN reached 77.27% and DN reached 69.59%. The AUC of IgAN was 0.733 and that of DN was 0.627. In addition, compared with renal pathologists, AI can distinguish IgAN from DN more quickly and accurately, and has higher consistency.DiscussionWe constructed an AI model based on Masson staining images of renal tissue to distinguish IgAN from DN. This model has also been successfully deployed in the work of renal pathologists to assist them in their daily diagnosis and teaching work.

Objetivo. Describir la prevalencia de la anemia entre pacientes con tratamiento de hemodiálisis crónica en una población de altura (3200 m s. n. m.), en el Departamento de Junín (Perú). Métodos. Estudio observacional, retrospectivo. Se tomaron los datos del Registro Nacional de Diálisis (RENDES, 2021) en el Departamento de Junín (Perú) y de las historias clínicas de los pacientes de hemodiálisis del Seguro Social EsSalud del año 2021. La población muestral estuvo compuesta por todos los pacientes que constituían el Programa de Hemodiálisis Crónica de la Región Junín. Se analizaron los datos descriptivos en Stata, versión 17.0. Resultados. La prevalencia de anemia fue del 3,9 %. Solo 5 pacientes presentaron anemia asociada a la enfermedad renal crónica (ERC) (3,9 %). Fueron recolectados datos de 128 pacientes. 64,8 % (n = 83) eran de sexo masculino. La principal causa de la ERC fue la hipertensión arterial (HTA) (37,5 %), luego la diabetes mellitus tipo 2 (24,2 %). El promedio de hemoglobina fue 12,8 ± 2,38 g/dL. El promedio de volumen corpuscular medio (VCM) fue 95,42 ± 4,9 fl, hemoglobina corpuscular media (HCM) 32,13 ± 1,9 pc, y la concentración de hemoglobina corpuscular media (CHCM) promedio fue 33 ± 1,15 gr/dL. El nivel de hierro sérico promedio fue 100 micromol/L y el nivel de ferritina en promedio fueron de 1300 ng / ml. Los pacientes utilizaban en promedio 23000 UI de eritropoyetina (EPO) mensual, 400 mg de hierro mensual y 4 mg de hidroxocobalamina mensual. Conclusiones. Las tasas de anemia en pacientes con tratamiento de hemodiálisis crónica en Huancayo, Junín, en una ciudad de altura, son bajas (3,9 %).

Objectives: This study was to explore the application value of B-ultrasound in guiding puncture biopsy of chronic kidney disease (CKD) and the clinical nursing effects under the guidance of B-ultrasound. Methods: Pathological examination of kidney biopsy was performed on 94 patients with CKD under the guidance of ultrasound from August 2020 to December 2020.; patients were observed for symptoms such as low back pain, backache, hematuria, and subcapsular hematoma. Color Doppler ultrasonography was performed on the punctured patients on day 1, 2, and 3 to observe whether there was subrental hematoma. The pathological results were analyzed and the success rate of percutaneous renal biopsy under ultrasound guidance was analyzed. Before the patient was discharged, investigate the satisfaction with the nursing work. Results: (1) After the puncture, 45 patients developed low back pain and low back pain symptoms, 12 cases developed subcapsular hematoma; 8 cases showed gross hematuria, 62 cases showed microscopic hematuria, and the rest had no obvious symptoms; (2) the nursing satisfaction rate of 94 cases was as high as 95.7%. Conclusion: US-PRB is a safe and effective auxiliary examination method, which can improve the success rate of puncture and reduce postoperative complications.
