In this scheme, the employee's contribution rate is 0.75% of the wages, and that of the employer's is 3.25% of the wages paid/payable in respect of the employees in every wage period, and though the contributions are based on the workers IntroductIonOral cancer is one of the commonest cancers in India, and as per the Globocan 2020 data, it is the most common cancer among Indian males and the second most common cancer in both sexes. 1 Though curable if detected in early stages and also easily accessible by virtue of site for examination, both by a patient as well as healthcare worker, most patients reach in locally advanced and late stages, leading to complex management protocols, poorer treatment outcomes, and increasing health care burden. 2 Most common risk factor is tobacco usage. [3][4][5] In India, rampant usage of various forms of smokeless tobacco like khaini, gutkha is the leading risk factor, others being alcohol consumption, low socioeconomic status, and lack of oral hygiene.This retrospective study was conducted in a tertiary care hospital and medical college and hospital of ESIC, which is a social security system aimed at the labor class population of India with a budget of 80,000 crore INR (US $11 billion). This scheme is one of the biggest social security providers in the world and is designed to provide socioeconomic protection to employees in the organized sector, with the provision of medical benefits being its major strength. We present a rare case of simultaneous occurrence of pleomorphic adenoma of the left parotid gland and squamous cell carcinoma (SCC) of left buccal mucosa in a 59-year-old patient. The synchronous occurrence of these two entities has not been reported in the literature. A PubMed database search did not yield any results for search words involving “Oral Cancer,” “Synchronous Oral Cancer and Pleomorphic Adenoma of Parotid gland” and “Oral SCC and Pleomorphic Adenoma of Parotid gland.” Furthermore, synchronous development of these two tumors may give rise to diagnostic and ma?nagement conundrums as the parotid growth may be considered to be a nodal metastasis.

Objetivo: O objetivo desse estudo foi avaliar a eficácia do diagnóstico e tratamento de lesões orais entre diferentes níveis de saúde. Material e Métodos: Todas as informações foram obtidas dos prontuários médicos de um Hospital Universitário e de fichas do Sistema Público de Regulação, considerando as características clínico-demográficas tal como, doenças orais diagnosticadas, sistema de referência e terapia empregada. A avaliação foi baseada na relação entre diagnóstico final, hipótese diagnóstica e prioridade para agendamento da consulta, também foi considerado o tempo decorrido entre o encaminhamento e o primeiro atendimento, data do diagnóstico final e início da terapia. Depois, a hipótese diagnóstica preenchida por profissionais de saúde do nível primário e terciário foram comparadas e a concordância entre ambos foi determinada. Resultados: Cerca de 51% dos encaminhamentos preenchidos pelos profissionais da atenção primária estavam incompletos, ou a hipótese diagnóstica estava descrita de forma inadequada. No nível terciário, o tempo médio entre o encaminhamento e a primeira consulta foi de 23 dias, a média entre o diagnóstico final e o início do tratamento foi de 22 dias. O tratamento cirúrgico foi o mais rápido (31 dias) quando comparado a radioterapia para lesões malignas (81 a 100 dias). Conclusão: É necessário melhorar o treinamento de profissionais de saúde da Atenção Básica no reconhecimento de lesões orais malignas. Claramente, o preenchimento adequado das hipóteses de diagnóstico, com o grau de prioridade correspondente acelera o diagnóstico e tratamento do paciente.
