Observational study of laryngoscopy plus flow‐volume loops during exercise

Abstract: Key Clinical MessageLaryngoscopy is the gold standard to diagnose exercise‐induced laryngeal obstruction, though inspiratory flow‐volume loop may provide a clue. We combined tidal flow‐volume loop analysis plus laryngoscopy during exercise and found that cigar‐shaped – not flattened – inspiratory loops are associated with obstruction. Pursed‐lip breathing slows inhalation thereby reducing vocal fold adduction.

“…A lack of correlation between resting spirometry and CLE findings has also been confirmed empirically 64 65. Exercise tidal flow-volume loops have not been extensively studied, but may prove useful in terms of demonstrating an attenuated inspiratory flow 66. Bronchial provocation tests (other than EIB tests) have been found to have limited value in EILO diagnostics, both when performed with and without a laryngoscopy in place 64 67.…”

“…Bronchial provocation tests (other than EIB tests) have been found to have limited value in EILO diagnostics, both when performed with and without a laryngoscopy in place 64 67. Eucapnic voluntary hyperpnoea (EVH) can induce glottic as well as supraglottic obstruction 66 67. Laryngoscopy performed during an eucapnic hyperpnoea test (EVH) would provide hyperventilation without exercising and could conceivably represent a diagnostic shortcut in the evaluation of EILO.…”

“…To můžeme pozorovat na flow--volume křivce, která má "cigaretový" tvar, což znamená, že velikost křivky průtok výrazně přesahuje velikost křivky objem. Zpomalením inspiračního průtoku například dýcháním přes sevřené rty lze obnovit normální eliptický nebo obdélníkový tvar křivky flow-volume a zmírnit síly způsobující addukci hlasivek (Pianosi at al., 2018).…”

“…CLE test umožní lokalizaci uzávěru laryngu, zobrazí také časování tohoto děje, a díky tomu lze zjistit, která ze struktur iniciuje addukci. V neposlední řadě určí tíži addukce (Pianosi at al., 2018;Tervonen et al, 2009).…”
