Long-term follow-up of lamellar macular holes and pseudoholes over at least 5 years

Abstract: LH, PH, and cleavedPH are often stable over a very long time. LH tends to worse visual function compared to PH and cleavedPH. A spontaneous separation of epiretinal membranes in the long-term is not uncommon. Vitreoretinal intervention should be considered in cases with significant visual loss or functional and morphological progression.

“…The rationale of the peeling of the CERM is the removal of traction; however, the rationale of the peeling of the AERM is controversial and intriguing. Some authors described LMHs with AERM as a stable condition, others demonstrated a slow but progressive evolution [4-14]. Furthermore, it has been hypothesized that AERM may be associated to Müller cell damage, and its surgical removal may cause the final loss of an already weak scaffolding property of Müller cells.…”

“…Taking this information into account, we have found that most authors recommend surgery when the patient complains of important metamorphopsia or there is a documented decrease in VA over time [16,27,36,38,52]. Other authors intervened with surgery whenever VA < 20/40 and there was evidence of ERM [41,42].…”

“…Most LMHs are morphologically stable over years of follow-up: 13% to 21% of cases show an enlargement of maximum LMH diameter and/or reduction in foveal thickness after 18-24 months [15,24]. After the longest published follow-up (8.3 years), 1/3 of cases presented morphological progression [27].…”
