Impact of baseline characteristics on outcomes of advanced HCC patients treated with sorafenib: a secondary analysis of a phase III study

Abstract: Within a cohort of highly selected advanced HCC patients, baseline patient-, liver-, and disease-centered variables play an important role in predicting patient outcomes. This information is important in terms of therapeutic decision-making and patient counseling.

“…Although CT-PMWA is a kind of minimal invasive therapies, preoperative hepatic function reserve was significantly associated with the long-term prognosis of patients. Our findings were consistent with previous studies [18][19][20][21][22][23][24][25][26]. Therefore, the better patients' hepatic function reserve was assessed, the better patients' prognosis could be evaluated.…”

“…Pinato et al have validated the ALBI grade in a large, multicenter and international study, and the ALBI grade was confirmed to provide objective hepatic reserve estimation across each Barcelona Clinic Liver Cancer (BCLC) stage of HCC [22]. In addition, the ALBI grade was suggested as a significant predictor for the OS of HCC patients who underwent surgical resection, transarterial chemoembolization (TACE), or sorafenib [22][23][24][25][26]. Kao et al constructed a nomogram with ALBI grade to assess the OS of patients with early-stage HCCs after RFA, in which study the personalized long-term OS data were offered [27].…”

“…In the sorafenib-based group, initially or non-initially treatment had no effect on OS and PFS. Several researchers indicated that proper therapies prior to sorafenib led to better survival outcomes for HCC patients (24,25). It implied that in the treatment strategy of advanced HCC, multiple combinations of locoreginal and systemic modalities treatment could be applied, which may have a beneficial therapeutic impact.…”

“…Multivariate analysis identified ALBI grade 1 as a significant strong predictor of a high ORR, and it is associated with the lowest probability of treatment discontinuation because of adverse events (AEs) [42]. High-grade AEs were observed more frequently in patients with ALBI grade ≥2 in the sorafenib group of the SUN1120 trial [43]. Also, patients with ALBI grade 1 receiving sorafenib show significantly better OS than those with ALBI grade ≥2 [44].…”
