Background: Esthesioneuroblastoma (ENB) is a rare sinonasal malignancy, lacking a unified staging system and treatment. Management at a single center was retrospectively evaluated to inform future treatment options and prognostic factors.Methods: Clinical data of 64 consecutive ENB patients, including prognostic factors and treatment methods, were reviewed retrospectively. Data were collected to calculate overall survival (OS) and progression free survival (PFS).Results: The majority of tumors 84.4% were within Kadish C stage, 79.7% were within T3 or T4, and 64.0% were within Hyams grade III or IV. A total of 50 (78.1%) patients received surgery and combined radiotherapy with or without chemotherapy, 10 (15.6%) received surgery with or without chemotherapy alone, and 4 (6.3%) received radiotherapy with or without chemotherapy alone. The majority of patients (79.7%) underwent endoscopic resection (endoscopic and endoscopically assisted). Surgery combined with radiotherapy with or without chemotherapy resulted in significantly better OS (84.4 vs. 50.6%, 84.4 vs. 37.5%) compared to surgery alone and radiotherapy alone (P = 0.0064). Endoscopic surgery group (endoscopic and endoscopically assisted) resulted in significantly better 5-year PFS (61.7 vs. 22.2%) compared to the open surgery group (P < 0.001). Although endoscopic surgery group was not a statistically significant predictor of 5-year OS (P = 0.54), the 5-year OS was 79.3% for the endoscopic surgery group and 76.2% for the open surgery group. A Cox regression analysis identified intracranial extension and surgery combined with radiotherapy as independent factors affecting 5-year OS while cervical lymph node metastasis and Hyams grade IV as independent factors affecting 5-year PFS.Conclusion: Our findings suggest that surgery combined with radiotherapy is the best treatment approach for ENB. For advanced tumors, endoscopic surgery is an effective treatment, and its survival rate is equal to or better than open surgery.

BackgroundTo evaluate the impact of postoperative radiotherapy (PORT) on survival in olfactory neuroblastoma (ONB) patients with different stages. MethodsPatients with ONB were selected in the Surveillance, Epidemiology, and End Results (SEER) database during 2004–2016. Survival analyses were performed using Kaplan Meier (K-M) method, Cox regression analysis, and competing risk model. ResultsA total of 513 patients were included in the study. Univariate and multivariate analysis results demonstrated that PORT was not an independent prognostic factor for overall survival (OS) of patients with modified Kadish stage A and B (p=0.699 and p=0.248, respectively). For C and D cases, patients who underwent PORT had significantly better OS than those who did not undergo PORT (p=0.03 and p< 0.0001, respectively). K-M curves illustrated that the 5- and 10-year OS rates according to radiotherapy (PORT vs. non-PORT) were 70.4% vs. 85.3% and 56.8% vs. 68.2% in stage C, respectively. For stage D patients, the 5-year OS rates were 42.6% and 70.7%, and 10-year OS rates were 29.5% and 53.4% in the PORT and non-PORT groups, respectively. The competitive risk model revealed that the 5-year cancer-specific cumulative mortality incidence decreased by 26.6% and the 10-year mortality incidence by 41.4% in patients with stage C who were treated using PORT; meanwhile, for patients with stage D who were treated with PORT, the 5- and 10-year mortality incidence reduced by 35.3% and 42.6%, respectively. Chemotherapy was not related to the prognosis of ONB (all p> 0.05).ConclusionsOur results indicate that PORT improved survival outcomes in ONB patients with modified Kadish stage C and D. However, for modified Kadish stage A and B cases, PORT may not affect survival. Chemotherapy was not recommended for ONB patients until more studies determine the role of chemotherapy.

The study objective is to summarize clinical experience in the treatment of esthesioneuroblastoma (ENB) accumulated by specials at N.N. Blokhin National Medical Research Center of Oncology.Materials and methods. We analyzed the data of 115 ENB patients who had undergone treatment between 1965 and 2019.Results. The 15-year overall survival rate was 1.2–1.7 times higher in patients receiving comprehensive treatment (47.7 ± 11.3 %) than in those receiving other types of therapy. In addition to that, comprehensive treatment ensured the highest rates of 3-year, 5-year, and 10-year relapse-free survival (49.3 ± 11.8 %), as well as the longest median survival (7.2 years). Chemoradiotherapy was effective in 34.8 % of patients with locally advanced ENB, while in 9 out of 45 patients (20.0 %), it resulted in partial response, which suggest that such treatment can control aggressive disease course and increase survival. Surgical treatment (alone or in combination with chemotherapy and radiotherapy) was indicated for 64 patients. However, only in 43 of them (67.1 %), it was performed as originally planned. Five patients (7.8 %) had partially radical surgeries and sixteen patients (25.1 %) had non-radical surgeries.Conclusion. The most effective treatment strategy for locally advanced ENB is a combination of surgery, chemotherapy, and radiotherapy. Chemoradiotherapy at the first stage followed by surgery was found to be an optimal treatment scheme. Tumor sensitivity to chemo- and radiotherapy affects both short-term and long-term treatment outcomes. Partially radical surgeries for locally advanced ENB are acceptable if conservative treatment is planned after operation. Treatment strategy should depend on the tumor spread (stage), grade, and proliferative activity, as well as patient’s age, somatic status, and comorbidities. Multivariate analysis has demonstrated that none of treatment methods decrease survival. The most significant factors negatively affecting the prognosis were as follows: T3–4 tumor, involvement of regional lymph nodes before treatment initiation, distant metastasis, grade IV tumor, and Ki-67 index >21 %.
