The unmet provision of allergy services in Hong Kong impairs capability for allergy prevention – implications for the Asia Pacific region

Abstract: There is a high and rising prevalence of many allergic diseases in the Asia Pacific, including Hong Kong, which is unmatched by a commensurate provision of clinical allergy services. In the last 3 years, the allergy profile in Hong Kong has increased due to greater public engagement and more frequent educational activities, scientific outputs and publication of local guidelines on allergy prevention, diagnosis and treatment. Two new drug allergy clinics have been established in public hospitals, and for the fi… Show more

“…This is likely an even greater problem in Hong Kong (HK) where an upsurge of MRSA, extended spectrum beta-lactamase (ESBL) producing Escherichia coli, carbapenem-resistant Acinetobacter baumannii (CRAB), multidrug-resistant A. baumannii (MRAB), and carbapenemase-producing Enterobacteriaceae (CPE) has been known for decades 15, 16. Despite the severe consequences of incorrect BL allergy labels, the limitations in testing capacity and testing costs remain a significant barrier to comprehensive testing 17, 18…”

“…Therefore, it is near impossible to diagnose a potential excipient allergy if the culprit excipient cannot be identified in the causative drug or vaccine ( 17 ). Furthermore, given the unmet provision of allergy services and specialists in Hong Kong, anaphylaxis survivors remain severely underinvestigated and underdiagnosed ( 18 , 19 ). For example, among Hong Kong adult patients, more than 10% of anaphylaxis survivors did not have an identifiable cause for their allergies and were mistakenly labeled with a diagnosis of multiple food allergies ( 20 ).…”

“…The contaminative functional sequences (non-specific for antigen recognition) may be derived from a hybridoma that is not absolutely monoclonal, which could be induced by more than one spleen cell fused with myeloma partners [ 36 ] or a spleen cell that has been fused but containing a rearranged allele in the excluded chromosomes, a so-called “allelic inclusion” [ 37 , 38 ], along with mutations during the long-term cultivation. The aberrant unproductive VH and VLκ chains may be derived from the myeloma fusion partners caused by Ig VH and VLκ allelic aberrant rearrangement [ 38 , 39 , 40 ].…”
