Imaging mimics of pancreatic ductal adenocarcinoma

Abstract: Pancreatic ductal adenocarcinoma is the most common primary malignancy of the pancreas. The classic imaging features are a hypovascular mass with proximal ductal dilatation. Different pancreatic pathologies can mimic the imaging appearance of carcinoma including other tumors involving the pancreas (pancreatic neuroendocrine tumors, lymphoma, metastasis, and rare tumors like pancreatic acinar cell carcinoma and solid pseudopapillary tumors), inflammatory processes (chronic pancreatitis and autoimmune pancreatit… Show more

“…These key imaging features are also critical imaging findings in pancreatic ductal adenocarcinoma (PDAC). Overlaps in imaging findings between PNEC and PDAC have been reported previously [ 7 ]. In a prior study, we found that 57% of PNEC was misdiagnosed as PDAC [ 6 ].…”

“…due to overlap of the imaging ndings. [31,32] In this study, we found that suspicious lymph nodes, abutment or encasement of the arteries around the pancreas, venous involvement, and the tumor size have no abilty to differentiate between CP and CP with PDAC. Many other clinical features such as the duration of symptoms before admission, jaundice, high-colored urine, diabetes mellitus, total bilirubin levels, and the carbohydrate antigen 19 − 9 (CA19-9), the carbohydrate antigen 125 (CA125) were insigni cant using logistic regression, which are in agreement with these in preivout studies.…”

“…Similar to PDAC, focal CP can lead to focal parenchymal atrophy or enlargement and appears as a hypovascular mass on imaging, accompanied by upstream ductal dilation and atrophy[ 35 ]. Imaging features that point towards an inflammatory cause include an unobstructed or smooth tapering pancreatic duct through the hypovascular mass (duct-penetrating sign), while PDAC usually demonstrates an abrupt or irregular obstruction (Figure 2 )[ 36 - 38 ].…”
