“Autoimmune(-Like)” Drug and Herb Induced Liver Injury: New Insights into Molecular Pathogenesis

Abstract: Idiosyncratic drug-induced liver injury (DILI) and hepatic injury due to herbal and dietary supplements (HDS) can adapt clinical characteristics of autoimmune hepatitis (AIH), such as the appearance of autoantibodies and infiltration of the liver by immune competent cells. To describe these cases of DILI/HDS, the poorly-defined term “autoimmune(-like)” DILI/HDS came up. It is uncertain if these cases represent a subgroup of DILI/HDS with distinct pathomechanistic and prognostic features different from “classic… Show more

“…Based on our patient characteristics, the patient was found to be in the "highly probable" category. A subset of DILI named "autoimmune-like DILI" has been identified and is difficult to differentiate from "autoimmune hepatitis (AIH)" due to the considerable overlap of clinical and laboratory features [7]. In the event of autoimmune-like DILI, ALT and AST return to normal soon after the discontinuation of the offending agent in most cases without the requirement for immunosuppressants.…”

“…In human patients, CH is a risk factor for some drug reactions. [283][284][285] D-penicillamine (D-Pen) is the Cu chelator of choice (Table 10).…”

“…The presence of autoantibodies associated with herbal medicines has been previously well described, which agrees with our findings. 6,25,40 We did not apply the Roussel Uclaf Causality Assessment Method (commonly known as RUCAM) to assess DILI causality, since this tool has not been validated in patients with concurrent acute liver injury. However, our biopsy findings inarguably confirmed a second insult, most probably CAM-related DILI due to the heterogeneity in biopsy findings, that skewed from the expected findings.…”
