Utility of the Nanosphere Verigene in the Identification of Bacteremia Isolates From the Burn Intensive Care Unit

Abstract: Reasons for lower sensitivity than previously reported are unclear and, on the basis of this limited retrospective review, further study in the BICU population is needed. The Verigene appears useful for antimicrobial stewardship by accelerating the identification of blood isolates.

“…Several additional items should be noted when interpreting the results. The improvement in time to appropriate therapy associated with BCID implementation in this study (absolute median reduction of 1 h) was significantly less than that of prior studies 2,4,5,7–9,16–18,21,22 . In a previous single‐center analysis, BCID implementation shortened time to appropriate therapy from 15 to 5 h without ASP and from 13 to 5 h with ASP 8 .…”

“…As reflected in the ASP survey results, most VA hospitals that have implemented BCID have high functioning ASP programs (e.g., nearly 80% perform blood culture reviews at 48 h). Therefore, it is possible that the enhanced ASP practices within the VA system over time accounted for the less pronounced effect of BCID implementation on time to appropriate therapy in this study relative to other studies of a similar nature 2,4,5,7–9,16–18,21 . This is potentially corroborated by the Desirability of Outcome Ranking for the Management of Antimicrobial Therapy (DOOR MAT) assessments of BSIs in the VA, which demonstrated improved trends in empiric antimicrobial prescribing from 2009 to 2018 23 .…”
