Measurements of wear resistance and metal ion release are important for designing bearing couples or interfaces in total hip arthroplasty (THA). In this study, we investigated wear resistance and metal ion release of surface-modified metal-free all-polymer hip bearings, such as poly(ether-ether-ketone), (PEEK) on cross-linked polyethylene (PEEK-on-CLPE), with a hydrated gel-like surface layer, to propose an improved alternative to the conventional materials used to design THA bearings. The PEEK surface resulted in less metal ion release than the cobalt-chromiummolybdenum (Co-Cr-Mo) alloy surface owing to the lack of metal. The PEEK-on-CLPE bearing (6.33 mg/10 6 cycles) had lower wear (rate) than the bearing with Co-Cr-Mo alloy-on-CLPE (10.47 mg/10 6 cycles) under controlled laboratory conditions; the wear performance of the all-polymer hip bearings was further improved with hemi-or both-surface modified with a hydrated poly(2-methacryloyloxyethyl phosphorylcholine) (PMPC) layer (3.74 and 3.06 mg/10 6 cycles, respectively). The PMPC-grafted interface of PEEK-on-CLPE will be especially suitable for THA candidates. This study is of key importance for the design of lifelong THA and a better understanding of the limitations resulting from using PEEK. Further studies are necessary to evaluate the possibility of using this material in artificial hips.

Resumo Objetivo: Verificar se implantes metálicos de uso rotineiro (aço inoxidável, liga de alumínio, cobalto-cromo-molibdênio e feitos de titânio) serão detectados em um aeroporto internacional do Brasil e gerar informações úteis para evitar transtornos ao paciente e apoiar os órgãos reguladores de segurança. Métodos: Estudo experimental, não randomizado, controlado e cruzado foi realizado recrutando dois indivíduos, um homem e uma mulher, para passar por um detector de metais padrão do aeroporto com implantes ortopédicos presos ao corpo. Foram testados implantes com diferentes composições, pesos e em várias partes do corpo. Resultados: De todos os implantes testados, não houve detecção de implantes para fixação interna, sejam de aço ou de titânio. O fixador externo foi detectado e a única diferença na composição é que o fixador externo testado é de liga de alumínio. Todos os implantes de artroplastia do quadril testados foram detectados. Dois implantes de artroplastia do joelho foram testados, e ambos eram feitos de cobalto-cromo-molibdênio, mas com especificações diferentes, e apenas um deles foi detectado. Conclusões: Neste estudo com implantes ortopédicos ex-vivo, verificamos que os implantes de osteossíntese compostos por aço inoxidável ISO 5832-1 não acionaram o arco detector de metais do aeroporto. No entanto, o fixador externo e as próteses articulares totais foram mais frequentemente detectadas.

Background: In era of increased air travel, airport security screening measures has been increased. A lot of anxiety to the patient, that may trigger an alarm at airport securities post-surgery. The purpose of this study is to find out experiences of patients after total hip arthroplasty (THA) passing through airport security. Methods: A retrospective case series of 269 THA patients in a single high-volume center from January 2016 to June 2018, who had passed through airport security and met inclusion criteria. Patient were contacted during their regular follow ups or via phone. The patients were asked for alarm trigger, perceived inconvenience, whether security officials asked to show documentation regarding prosthesis, and any extra screening procedures check measures.Results: Out of 269 patients, 52 patients met inclusion criteria travelling by airplane. 27 patients reported alarm trigger. 7 patients had to undergo additional security check measures. 43% of the patients believed that having their THA increased the inconvenience while traveling. Conclusions: This study provides information to surgeons regarding airport travel post THA. Patients can be counselled regarding the inconvenience and to be prepared for delays in airport and to be prepared to present documentation of their prosthesis.
