Long-term effects of a renin inhibitor versus a thiazide diuretic on arterial stiffness and left ventricular diastolic function in elderly hypertensive patients

Abstract: Arterial stiffness and cardiac function are important predictors of cardiovascular events in patients with hypertension, even with adequate blood pressure (BP) control. We evaluated whether a direct renin inhibitor, aliskiren, reduces arterial stiffness and modulates left ventricular function compared with a diuretic, hydrochlorothiazide, in elderly hypertensive patients. Twenty-one hypertensive patients [67 ± 14 (SD) yr] were randomly assigned to receive 6-mo aliskiren ( = 11) or hydrochlorothiazide ( = 10)-b… Show more

“…As can be seen from Figure 7 , aliskiren did not affect LV SV, EDV and ESV, which is consistent with clinical trials ( Solomon et al, 2011 ; Okada et al, 2017 ). In our case, EF also did not change in treatment with aliskiren, in accordance with a number of experimental studies ( McMurray et al, 2008 ; Solomon et al, 2011 ; Pitt et al, 2011 ).…”

“…However, since aliskiren has the possibility to increase the risk of hypotension and hyperkalaemia, it may be expected to apply for HF patients who are suffered from hypertension or hypokalaemia at the same time [32]. Moreover, a small-scale published RCT has initially shown that aliskiren lowered arterial stiffness and left ventricular diastolic function in elderly hypertensive patients during a follow-up of 6 months [33].…”

“…Firstly, a RAAS inhibitor may not suit all heart failure patients. Since aliskiren has been associated with adverse events such as hypotension and hyperkalaemia, it may be best indicated for the heart failure patients who have a past history of hypertension and laboratory examination of hypokalaemia [ 18 ]. Most studies of our meta-analysis have included heart failure with reduced ejection patients.…”
