IntroductionThe optimal duration of dual anti-platelet therapy (DAPT) following percutaneous coronary intervention (PCI) is still a matter of debate. Biomarkers may help to identify patients who will benefit from extended DAPT. The aim of the study was to test the interaction between lipid parameters and platelet function in patients with coronary artery disease (CAD) on DAPT.Material and methodsOverall, 58 patients on DAPT were prospectively included following PCI in stable CAD. Platelet markers, i.e. mean platelet volume (MPV), platelet distribution width (PDW), fraction of reticulated thrombocytes (RT) and ADP-induced multiple electrode aggregometry (MEA), as well as serum lipids, i.e. high-density lipoprotein cholesterol (HDL-C), low-density lipoprotein cholesterol (LDL-C), total cholesterol (TC), triglycerides (TG) and remnant cholesterol (RC), were assessed after intake of a maintenance dose of ASA and P2Y12 inhibitor.ResultsA significant inverse correlation was found for HDL-C levels and markers of platelet activation: MPV (r = –0.351, p = 0.009), PDW (r = –0.391, p = 0.003), fraction of RT (r = –0.402, p = 0.003) and ADP-induced MEA (r = –0.345, p = 0.011). Only a weak or no association was found between other lipid parameters and platelet markers. After multivariable adjustment, HDL-C levels served as a strong and significant predictor of MPV (95% CI: –0.039 to –0.009; p = 0.002), PDW (95% CI: –0.094 to –0.034; p < 0.0001), RT (95% CI: –0.107 to –0.031; p = 0.001) and MEA (95% CI: –0.540 to –0.170; p < 0.0001), while TG, LDL-C, RC and TC were not significantly associated with platelet function.ConclusionsWithin lipid parameters, only HDL-C levels are strongly associated with markers of platelet activation in CAD patients on DAPT. Accordingly, detection of dyslipidemia might indicate the need for prolongation of DAPT.

Introduction: Non-thyroidal illness syndrome (NTIS) is one of the signs for poor prognosis of cerebral infarction (CI), but its risk factors had never been explored. In this study, we analyzed the potential effect of collateral circulation on prognosis prediction of triiodothyronine for large artery atherosclerosis cerebral infarction (LAA-CI) patients. Material and methods: Clinical data of CI patients between 2012 and 2014 were collected. Imaging inspection was used for determining TOAST classification and evaluating collateral circulation. One-year follow-up was conducted for mRS score by telephone. Results: T3 level in the NTIS group (p = 0.001) was significantly decreased while TSH level (p < 0.001) was increased. Patients in the NTIS group had a poorer prognosis (p = 0.008) and the main reason was the high mortality (p = 0.002). NTIS predicted poor collateral circulation (p = 0.026) and good collateral circulation tended to be less likely concomitant with NTIS (p = 0.001). Logistic regression analysis showed that triiodothyronine concentrations (OR = 4.760, 95% CI: 1.981-11.456, p < 0.001) were positively correlated with but advanced age (OR = 0.756, 95% CI: 0.645-0.886, p = 0.001) negatively with opening of collateral circulation. Conclusions: Poor opening of collateral circulation was likely to mediate the prediction of NTIS for prognosis of LAA-CI patients.

Introduction: Although several studies have been conducted on the association of mean platelet volume (MPV) with the diagnosis, morbidity, and mortality of coronavirus disease 19 (COVID-19) patients, the results were contradictory. We aimed to investigate the effects of MPV on hospitalization and mortality with a significant number of patients. Methods: This multicenter study was carried out retrospectively on 9.487 patients. The demographic information and laboratory parameters of the patients were obtained from their medical records in the hospital. Analyses were performed using Statistical Package for the Social Sciences (SPSS) version 22.0 for Windows. Results: In total, there were 806 intensive care unit (ICU) patients, in addition to 8.681 non-ICU patients. The mean age of patients in ICU was significantly higher than non-ICU (71.25±13.42 vs. 56.17±16.59; p<0.001). MPV values at admission and max MPV values during the period of hospitalization were significantly higher in patients followed in the ICU (p<0.05). MPVfirst , MPV last , ΔMPV last-first , and ΔMPV% values were significantly higher in non-survivor group than the survived patients. As a result of receiver-operating characteristic analysis, the risk of mortality was determined to be 5.15 fold greater in people with MPVlast ≥10.05 fl. (ODDS ratio:5.15 95% CI: 4.374-6.067). The patient has a 4.62 fold risk of death after a 2.18% increase of arrival MPV value (ODDS ratio: 4.62, 95% CI: 3.455-6.203). Discussion and Conclusion: We thought that MPV and MPV changes during hospitalization may be a powerful predictor of mortality in patients with COVID-19.
